Baroreflex sensitivity (BRS) is a primary mechanism for acute and chronic control of blood pressure (BP).However, there are few data showing the relationship between BRS and ambulatory BP (ABP). We assessed the hypothesis that BRS specifically contributes to some specific parameters of ABP in never-treated hypertensive/normotensive subjects. We studied 128 subjects (mean age: 54.5 ± 13 years, 60% male) consisting of 92 untreated hypertensive and 36 normotensive subjects. Radial tonometric BP and simultaneous RR interval were recorded for 10 min, and the Valsalva maneuver was performed 3 times for each subject. BRS was calculated in two ways: the spontaneous-BRS by the spectral method, and the Valsalva-BRS by the slope method, using commercial software. ABP monitoring was performed on the same day as the BRS test.

The objective of this study was to examine the effects of dihydropyridine calcium antagonist therapy on 24-h baroreflex sensitivity. Twenty-three patients with moderate essential hypertension were studied before and during acute (10 patients) and chronic (21 patients) treatment with a dihydropyridine calcium antagonist (nifedipine, nicardipine or felodipine) as monotherapy in a dose titrated to produce a fall in mean cuff pressure of at least 10%. Twenty-four hour unrestricted ambulatory intra-arterial blood pressure (IABP) and heart rate (R-R interval) were monitored. Baroreflex sensitivity (BRS) was assessed throughout the 24-h period by off-line computer analysis of spontaneous variations in IABP and R-R interval. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : This study included 214 hypertensive patients, under antihypertensive medication. All study participants underwent 24-hour ambulatory blood pressure monitoring (ABPM) and maximal exercise testing. Thereafter patients were divided into two groups: 94 dipper hypertensives and 120 non-dipper hypertensives. Heart-rate (HR) response during exercise was evaluated by the chronotropic index (CI). HR recovery (HRR) was defined as the difference between HR at peak exercise and first minute after the exercise test. R Re es su ul lt ts s: : Daytime systolic and diastolic BP measurements were similar, however night-time measurements were significantly lower among dippers than nondippers (night-time systolic BP: 114.1±11.4 vs 126.3±15.0 mmHg, p<0.001; night-time diastolic BP: 72.3±7.3 vs 79.9±9.8 mmHg, p<0.001). Chronotropic index was lower than normal in both groups (0.79±0.1 vs 0.78±0.7 p=NS). Dippers had higher HRR values than non-dippers (31.8±4.6 vs 29.1±4.6 p<0,001). HRR was positively correlated with the percentage decline of systolic and diastolic BP from day to night (r=0.255 p<0.001 and r=0.228 p=0.001, respectively). There were no correlation between CI and percentage of systolic and diastolic BP reduction from day to night (r=0.067 p>0.05 and r=0.16 p>0.05, respectively).
