Skull, Patella and Thigh Cryptococcosis after a Crashing Injury of the Temporal Bone

Abstract: An unusual case of triple location (skull, patella and thigh) Cryptococcus neoformans is described. The peculiarity of the case is based on the probability of direct post-traumatic bone inoculation, subsequent seeding from skull to patella, thigh and cerebral spinal fluid (CSF), patella involvement, lack of evidence of lung involvement and lack of predisposing underlying disease. The response to surgery and a combination of amphotericin B and 5-flucytosine administration was favorable without relapse at follow… Show more

“…Multifocal skeletal lesions were seen in two patients: skull/patella in one and frontal/mandible in the other [152,153]. A combination of skull and lung infection was also reported by Luhr and Svane [147].…”

“…To date, skull involvement has been reported in less than 20 cases [139][140][141][142][143][144][145][146][147][148][149][150][151][152][153][154][155]. Mechanisms involving cryptococcal infections of the skull include disseminated infections, skull trauma, and embolic phenomena [146].…”

“…These spores are a cause for cryptococcal infection (Armonda et al, 1993 ; Wood and Miedzinski, 1996 ). Direct inoculation during trauma is also possible (Chleboun and Nade, 1977 ; Dounis et al, 1991 ; Italiano et al, 2001 ). However, only 14.8% of the patients included in this review had contact with soil or pigeons or a history of trauma; incomplete patient histories might explain this finding.…”

“…The impact of probiotic Bacillus strain has also been reported on distal organs. For instance, the spores of B. clausii (Enterogermina ® ) were shown to enhance the production of IFN-γ in murine spleen cells, rabbits and mice animal models ( Muscettola et al, 1991 , 1992 ; Kosak et al, 1998 ). Vegetative cells of B. firmus have been shown to stimulate the proliferation of human peripheral blood lymphocytes in vitro ( Prokesova et al, 1994 ).…”
