Skin testing versus radioallergosorbent testing for indoor allergens

Abstract: Background: Skin testing (ST) is the most common screening method for allergy evaluation. Measurement of serum specific IgE is also commonly used, but less so by allergists than by other practitioners. The sensitivity and specificity of these testing methods may vary by type of causative allergen and type of allergic manifestation. We compared ST reactivity with serum specific IgE antibodies to common indoor allergens in patients with respiratory allergies.

“…However, some sensitized subjects have an IgE level below this cutoff, and the measurement of serum specific IgE is usually less sensitive than SPTs. 295 Although a low specific IgE titer may not be clinically relevant, the titer of serum specific IgE is usually unrelated to the severity of symptoms.…”

“…These tests are useful in pinpointing the offending allergen(s) and making a definitive diagnosis. Skin prick testing (SPT) with aeroallergens common to a particular geographic area is a widely accepted, safe, convenient and cost effective [37] procedure, and one of the most widely used diagnostic screening tests [38, 39]. SPT is less useful in conditions involving generalized eczema, dermographism, in individuals with recent uses of medications that have significant antihistaminic properties like tricyclic antidepressants, or undergoing high dose corticosteroid treatment.…”

“…However, some sensitized subjects have an IgE level below this cut-off. The measurement of serum-specific IgE is usually less sensitive than skin prick tests [43] and the worst correlations between SPT and IgE assays are obtained with mold, food extracts and non standardized extracts. In general, the correlation between a strongly positive response to a skin test and the detection of serum-specific IgE and between a negative response to a prick test and the lack of detection of serum-specific IgE is very good.…”
