IntroductionSkin-reducing mastectomy has been applied to several surgical techniques in which subcutaneous mastectomy is associated with various types of skin reduction, with preservation of a lower dermal flap to reinforce the inferior lateral seat of an implant. The aim of the study is to present a case series of patients with pendulous/ptotic and/or large-sized breasts treated for breast cancer at the Breast Surgery Unit of Istituto Nazionale Tumori IRCCS Fondazione “G. Pascale”, Naples, Italy, with the superomedial pedicle skin-reducing mastectomy technique, two-stage reconstruction, and transaxillary video-assisted technique, when a postoperative radiotherapy was indicated. We verified its effectiveness by discussing its results, especially in patients who are candidates for postmastectomy radiotherapy.Materials and methodsA single-center retrospective study was performed between January 2020 and March 2021 on a prospectively filled database of conservative mastectomies. Of the 64 patients who underwent nipple/skin-sparing mastectomies in the mentioned period, 17 (mean age 46 years, range 30–62 years) were treated with superomedial pedicle skin-reducing mastectomy, with two-stage breast reconstruction through transaxillary video-assisted replacement expander with definitive prosthesis and contralateral symmetrization, selected for postmastectomy radiotherapy.ResultsWe had only three minor complications. No flap necrosis, no infections, no breast seromas, and no reconstructive failures were observed. During follow-up of the patients treated with video-assisted reconstruction, there were no cases of infection, hematoma, implant rupture, or suture dehiscence in the reconstructed breast.DiscussionSkin-reducing mastectomy with superomedial pedicle is a safe and reliable procedure to treat breast cancer in selected patients, i.e., those with pendulous/ptotic and or large-sized breasts. Particularly, in patients who undergo postmastectomy radiotherapy, the two-stage reconstruction with video-assisted transaxillary endoscopic approach can find its main indication, using incisions positioned far from the mammary region, offering numerous advantages.

One of the largest organs of the human body is the skin and its pigmentation differs among the population. During skin disease identification, the dermatologist requires a high level of expertise and accuracy. This study proposes different kinds of skin image feature extraction and classification methods. In this work, we have chosen six kinds of skin diseases such as melanoma, seborrheic keratosis, eczema, actinic keratoses, nevus, and lupus erythematosus. In the preprocessing stage, the color standardization is performed by gray world color constancy (GWCC) algorithm. The thick and thin hairs from the disease images are removed in the preprocessing stage. The circular kernel with morphological operation clearly segments the skin lesion region. Moreover, the combination of novel cumulative-based level difference mean (NCLDM) and improved Asymmetry, Border Irregularity, Color Variation and Diameter (ABCD) features vector (ABCD-fv) methods is more helpful to extract the shape, texture, and color feature of skin lesion. However, the more features never offer an accurate classification result, so we go for the ranking and selection of features. Finally, the improved gray wolf-based multiple-layer perceptron (IGWO-MLP) technique is used to produce the relevant skin disease class. For the experimentation, there are six datasets such as DermNet, Xiangya, Medicine Net, PH 2 , Kaggle, and HAM-10000, which are chosen for effective skin disease identification. The proposed method demonstrates better segmentation, feature extraction, and classification result in terms of accuracy, specificity, sensitivity, Jaccard similarity index, and Dice similarity index. As a result, the skin disease identification of the proposed IGWO-MLP method yields 98% accuracy, 99% sensitivity, 98% specificity, 98% Jaccard Similarity Index, and 99% Dice similarity index when compared with state-of-the-art methods. This work will certainly help dermatologists to make their work more efficient and help them to provide the correct treatment for the skin disease detected.

Analysis of Trending Topics in Breast Cancer Articles From an Altmetric Perspective

Background It is widely known that social media has an impact on politics and the economy. The Altmetric Attention Score (AAS) is a new Web-based metric that was recently developed for use in the scientific field. The objective of this study was to assess which recent studies on the topic of breast cancer received the most attention from the general public. Methodology An Altmetric Explorer search was performed on January 7, 2022, to extract the following information: journal name, impact factor (IF), year of publication, article topic, article type, and level of evidence. Results The journal that published articles that received the most attention on social media was the New England Journal of Medicine (n = 8). All of the articles were published in journals in the highest IF quartile. The most frequent top three subjects in the top 50 articles were “treatment and management,” “risk factors for breast cancer,” and “breast cancer screening.” The number of articles with a level of evidence of 1, 2, 3, and 4 was 12, 17, 17, and 4, respectively. The correlation between AAS and citation was not significant. Conclusions The AAS seems to be a more reliable assessment of public perception of breast cancer. We propose that combining the AAS and traditional metrics may provide a more detailed description of scientific research output.
