Six Years' Experience in Treatment of Chromomycosis with 5‐Fluorocytosine

Abstract: Twenty-three patients with chromomycosis were treated with oral 5-fluorocytosine for periods ranging from 2 to 67 months. Sixteen were considered cured, after 3 consecutive monthly controls showing healing of the lesions and absence of fungi in mycologic and histopathologic examinations of biopsy specimens. Seven, in spite of initial clinical improvement, showed resistance to treatment. Associated treatment with amphotericin B, calciferol or thiabendazole failed to help patients resistant to 5-fluorocytosine. … Show more

“…Chemotherapy was used for long‐standing, very extensive cases with variable results. In most cases 5‐fluorocytosine and amphotericin B (intravenous) resulted in an important reduction of lesions and in cure in only one case for each drug; these results being similar to those reported in the literature [20–23]. Ketoconazole was later used at 200–600 mg day −1 doses, either as monotherapy or with calciferol (vitamin D2), with variable results.…”

“…Fluorocytosine (converted into 5-fluorouracil in fungal cells) shows some degree of efficacy but is associated with high risk of development of resistance, besides being hepatotoxic and myelotoxic. 123 With the emergence of more recent antifungals, the drug is now rarely used except in selected resistant cases.…”

“…26 With the emergence of newer antifungals it is rarely used nowadays except for resistant cases. Amphotericin B monotherapy is ineffective, and even in combination with other antifungals, results are generally poor, but a combination of amphotericin B and flucytosine has shown efficacy, with in vitro studies indicating synergistic activity between the two drugs.…”
