Sir Victor Horsley's 19th century operations at the National Hospital for Neurology and Neurosurgery, Queen Square

Abstract: Although he was not the first man to operate on the brain, Sir Victor Horsley was the world's first surgeon appointed to a hospital post to perform brain surgery, which happened in 1886 at the National Hospital for Neurology and Neurosurgery, Queen Square, London. The authors examined the patient records between 1886 and 1899 and found 151 operations performed by Sir Victor Horsley at the National Hospital, including craniotomies, laminectomies, and nerve divisions. The authors present the outcome data and cas… Show more

“…Victor Horsley (1857-1916), probably the most important neurosurgeon of his time, was the first to use bone wax to stop bleeding after trepanation and craniotomy and created the first assumptions of the future stereotaxic method 43 . He was also the first to successfully surgically remove a spinal extramedullary tumor after a laminectomy in 1887 43 .…”

“…For a long time in the past, there were many misunderstandings about epilepsy, which were associated with religion or culture (Ali et al 2016 ). It was not until 1886 when Victor first reported on surgical treatment for post-traumatic epilepsy by performing a craniotomy, which is considered the first epilepsy surgery in history (Singh and Dua 2023 ; Uff et al 2011 ). Psychological and behavioral methods were also applied to observe and assess epilepsy (Shafran et al 2020 ; Haut et al 2019 ; Cross 2015 ).…”

“…Awake craniotomy has been adopted for the surgical treatment of intractable epilepsy more than 100 years ago, and is now considered as a gold standard of care for resection of tumors, such as primarily gliomas, located within or close to the eloquent areas (i.e., sensorimotor or language areas) of the brain [ 1 , 2 ]. The modern awake craniotomy techniques have evolved in combination with intraoperative neurophysiologic monitoring to identify the eloquent areas of the brain.…”
