“…The group from Erasmus Medical Center Cancer Institute has previously assessed dosimetric feasibility of focal vocal cord irradiation for early‐stage local cancer T1aN0 using 4D‐CT; they showed sparing of contralateral vocal cord and various laryngeal structures compared to conventional radiotherapy . The same group recently published the first clinical outcomes from a retrospective analysis of 30 patients with T1aN0 glottic cancer treated with single vocal cord to a dose of 58 Gy in 16 fractions and showed excellent 2 year LC of 100% . In that study, CTV to PTV margin was defined as an isotropic margin of 3 mm for the first five patients, and was then increased to 5 mm based on observed intra‐fraction target motion for the remaining patients.…”

Our prospective study is limited by the fact that no clinical outcomes using the proposed margins are reported.…”

“…Newer treatment approaches utilize intensity modulated radiotherapy to treat the larynx while sparing the adjacent carotid arteries and achieves 88% local control rate at three years for T1–2N0 glottic SCC . This highly accurate localized treatment has been taken one step further by a Dutch group who limited radiotherapy to the single involved vocal cord and reported a two‐year local control rate of 100% . Some of these T1/T2 patients will not be cured by radiotherapy and some will subsequently undergo salvage total laryngectomy (STL‐LOR).…”
