Single strip lesions radiofrequency denervation for treatment of sacroiliac joint pain: two years’ results

Abstract: Background: Sacroiliac joint pain can be managed by intra-articular injections or radiofrequency of its innervation. Single strip lesions radiofrequency denervation is a new system. The objective of this study was to present one of the first utilizations of this innovative technique. Methods: 60 patients who met the diagnostic criteria for sacroiliac joint syndrome were enrolled in the study. In total, 102 single strip lesions radiofrequency denervations were performed. Pain intensity was measured with the Osw… Show more

“…Sensory stimulation in the SIJ at 50 Hz was performed to elicit paresthesia under 0.8 V, while to verify no motor recruitment in the ipsilateral lower extremity, motor stimulation was conducted at 2 Hz and at up to 1.5 V. Denervation by CRF (Avanos Medical Inc., Alpharetta, GA) was performed for 150 seconds at 60℃, with three lesions created at S1 and S2, and two at S3 [24]. The needle was positioned between the junction of the sacral ala and the S1 superior articular process to lesion the L5 dorsal ramus [24,[29][30][31]. After each procedure, the patient was observed for at least four hours before being discharged.…”

“…Eine nachhaltigere Alternative zur Infiltration stellt die gekühlte Radiofrequenzablation der zuführenden Nervenbündel zum ISG dar [33,34]. Sie stellt eine Weiterentwicklung der konventionellen und gepulsten Radiofrequenzablation dar [35][36][37]. Ergänzt werden sollte die medikamentöse Therapie durch physikalische Therapiemaßnahmen wie Physiotherapie mit einem individuell ausgewählten Trainingsprogramm [38] bzw.…”

“…Most studies assessed response to diagnostic injection within hours whereas some assessments occurred at their next scheduled appointment, which could have occurred as far out as 6 months postintervention . Five studies did not define when they assessed response to diagnostic injection . Some of this variability related to assessment of corticosteroid effect rather than local anesthetic effect.…”

“…The percentage of relief required for a diagnostic response to be considered positive varied: 80%, 12,14,15 75%, 9,[16][17][18] and 50% in the remaining studies were defined as thresholds, except for three studies in which the required percentage of pain relief was not specified. [19][20][21] In one sham randomized controlled trial, patients were eligible for randomization if they had pain reduction of two or more points on the numeric rating scale (NRS) with one diagnostic, intra-articular anesthetic injection. 22,23 Patients were selected for treatment in 18 of 32 studies following only one single diagnostic block.…”
