Single-Stage Anterior Debridement and Fibular Allograft Implantation Followed by Posterior Instrumentation for Complicated Infectious Spondylitis

Abstract: Complicated infectious spondylitis is an infrequent infection with severe spinal destruction, and is indicated for combined anterior and posterior surgeries. Staged debridement and subsequent reconstruction is advocated in the literature. The purpose of this study is to evaluate the feasibility and clinical outcome of patients who underwent single-stage combined anterior debridement and fibular allograft implantation followed by supplemental posterior fixation for complicated infectious spondylitis. We retrospe… Show more

“…Still, thirty percent of patients had throracolumber junction lesions, so this assessment was included for comparison purposes. Finally, the positive total positive culture rate was 71.4% and the intra-operative positive culture rate was low (52.4%) compared to other studies [5,16,42]. This lower positive culture rate may be related to the fact that our hospital is a tertiary care center, and some of the referred patients had been receiving parenteral antibiotics for a period of time before cultures were obtained.…”

“…Posterior spinal surgeries for implantation procedures are increasingly performed for a variety of spinal disorders. 3,6,7,24,28 Although most spine surgeons are familiar with pedicle screw instrumentation via the posterior approach, pedicle screw misplacement remains possible even under intraoperative fluoroscopic assistance. Numerous pedicle screw instrumentation-related complications, both intraoperative and postoperative, have been reported.…”

“…Various biological and mechanical spacers, including fibular autograft, fibular allograft, and even antibiotic-impregnated methylmethacrylate, are used to reconstruct the anterior column after corpectomy[ 18 - 21 ]. Antibiotic-impregnated methylmethacrylate is sometimes formed into antibiotic beads to salvage deep wound infection in our clinical practice.…”
