Single-port versus multiport laparoscopic ileocecal resection for Crohn's disease

Abstract: Single-port laparoscopic ileocecal resection is safe and feasible in Crohn's disease and is associated with less need for pain medication postoperatively as opposed to multiport laparoscopic ileocecal resection.

“…A laparoscopic approach over laparotomy is preferred, due to faster recovery and less pain with comparable rates of surgical recurrence 158-161 . Novel techniques include single port laparoscopic surgery, which could further reduce pain compared to conventional laparoscopy 162 .…”

“…This series, to our knowledge, represents the single largest sequential cohort experience focused solely on patients with CD to date. 24 By commencing cases on a consecutive basis (in the absence of a clear contraindication to laparoscopy), selection bias was avoided and disease complications such as fistulation, obstruction and abscesses (which others have excluded ab initio [5][6][7] and which only have been previously reported exceptionally 25,26 ) were included so reflecting routine practice. The majority of cases could be completed by SPLS alone although, interestingly, all those not so concluded were completed by laparotomy.…”

“…28 It allows the entire operation to be completed through one incision which is also used as the extraction site. 29 Mizushima et al reported 24 patients with structuring and perforating disease who underwent SILS for ileal resection, ileocecal resection, stoma closure, and stricturoplasty. Perioperative outcomes were found to be comparable besides blood loss, which was higher in the perforating disease group.…”

“…Perioperative outcomes were found to be comparable besides blood loss, which was higher in the perforating disease group. 28 According to other studies comparing SILS with conventional laparoscopy, it is safe and feasible in patients with complex Crohn's disease, resulting in less need for pain medication, 29 shorter operative time, 30 and with technical advantages over multiport technique. 31 Single incision technique can be the procedure of choice especially in pediatric population and adolescent patients who have an indication for diverting ileostomy.…”
