Single port laparoscopic right hemicolectomy with D3 dissection for advanced colon cancer

Abstract: We report the first case of single port laparoscopic right hemicolectomy for advanced colon cancer. An abdominal 3 cm length incision was made via the umbilicus. A small wound retractor and a surgical glove were used as a single port. All soft tissue anterior to the superior mesenteric vein was completely removed and D3 lymph node dissection was achieved. The total operative time was 180 min with minimal blood loss (< 50 mL). The size of the tumor was 5 cm x 3 cm and its tumor stage was T3N0. Sixty-nine lymph … Show more

“…SILS can be performed using refinements of existing technology, and surgeons can perform SILS without any new instruments, specific competence, or training. In the same way, single-incision laparoscopic colectomy (SILC) for colon cancer has been described through case reports and small case series [2][3][4]. This technique results in improved cosmesis with the potential for decreased pain and fewer incisional hernias.…”

“…Homemade transumbilical port using Alexis wound Retractor®, glove and conventional laparoscopic trocars have been in use to perform various operations like varicocelectomy, transumbilical pre-peritoneal (TAPP) inguinal hernia repair, hemicolectomy and laparoscopic total hysterectomy [1][2][3].…”

“…It can also be performed using one of the many commercially available multichannel single-port devices: R-port™ (Advanced surgical concepts, Dublin, Ireland), Uni-X™ (Pnavel system, Morganville, NJ, USA), and SILS port™ (Covedien). Several successful general surgical, urological and gynaecologica procedures were also performed with a homemade transumbilical port using the Alexis wound retractor® and surgical gloves [1][2][3]. The commercially available multichannel single-port devices are very costly and Alexis wound retractor® is not available in this part of our country.…”

“…Single-incision surgery is a rapidly evolving technique in the field of minimally invasive surgery, but only more recently [2][3][4] has this technique been applied to colorectal surgery. Single-incision laparoscopic colectomy (SILC) has been described through case reports and small case series [5][6][7]. The single-incision technique results in improved cosmesis with the potential for decreased pain and fewer incisional hernias.…”
