Single dose pharmacokinetics of lamivudine in subjects with impaired renal function and the effect of haemodialysis

Abstract: Aims The purpose of this study was to investigate the pharmacokinetics of a single oral dose of lamivudine administered to subjects with renal impairment and to determine whether lamivudine was dialysable in subjects with severe renal impairment undergoing haemodialysis. Methods Twenty-nine subjects were enrolled, nine with normal renal function (creatinine clearance (CL CR ) 82-117 ml min −1 ), eight with moderately impaired renal function (CL CR 25-49 ml min −1 ), six with severe impairment (CL CR 13-19 ml m… Show more

“…Lamivudine has a short serum half-life (5-7 h), although much of the drug is transported intracellularly and phosphorylated, the active metabolite has a half-life of 12-18 h [23]. The dose of lamivudine should be adjusted in patients with renal impairment [24]. Active lamivudine metabolites are not concentrated in the mitochondria, which may account for the low incidence of mitochondrial toxicity of lamivudine.…”

“…We believed that it was important to perform our evaluations at steady state because single-dose studies may fail to recognize the contributions of the drug distribution in tissue or alternative routes of metabolism or elimination induced by prolonged exposure to a drug. For example, in addition to renal elimination, lamivudine is also metabolized through a hepatic pathway (12). Although in patients with normal renal function hepatic metabolism contributes little to the overall metabolism of the drug, this pathway becomes more important in patients with renal impairment (12).…”

“…where Q H is the blood flow rate through the dialysis filter, H is the hematocrit, K is the concentration of lamivudine in red blood cells/concentration of lamivudine in plasma reported previously (12), and C a and C v are as defined above. Since in vitro studies indicate that lamivudine partitions equally between red blood cells and plasma (12), a value of 1 was used for K. The amount of lamivudine removed during hemodialysis was estimated by the following formula:…”
