Single Centre Results of Total Endovascular Repair of Complex Aortic Aneurysms with Custom Made Anaconda Fenestrated Stent Grafts

Abstract: "Real world" feasibility of the fenestrated Anaconda stent graft is demonstrated with 4% peri-operative mortality. Target vessel patency is in line with other series; however, limb thrombosis during follow up is of concern.

“…Despite failed catheterization, none of these patients required further reintervention or dialysis. The reported high success rate for catheterization of fenestrations/branches is in line with that reported with other multi-branch [13][14][15] and fenestrated [16][17][18] devices. Our peri-procedural complication rate and 30-day mortality are broadly comparable with the available literature [14,19], but it should be noted that most of our patients (42.8%) had previously undergone an aortic surgical/endovascular intervention.…”

“…Primary treatment was shown to have an early complication rate of 19% to 24%. [20][21][22][23] Postoperative complication risk and mortality require limitation of the use of the Fenestrated Anaconda cuff to patients in whom open repair is not an option, and careful selection of patients is essential.…”

“…Nevertheless, the wrinkles and folds on the surface of the graft should be expected to have an effect on the flow, altering the shear stresses or inducing micro recirculation, effects that have been connected to the formation of thrombus. 34 Aortic endografts have higher rates of occlusion than open surgical alternatives, 21,31 possibly because of the occurrence of folds within an oversized stent-graft. An occlusion within the limb of an aortic endograft is clinically significant as it can result in symptoms such as claudication, weakness and ischemia in the lower extremities.…”
