Impact of hospital readiness on patient safety incidents during the COVID-19 pandemic in Indonesia: health worker perceptions

ObjectivesThis study examined the impact of hospital readiness on patient safety from the healthcare workers’ perspective.DesignThe study employed a mixed-methods explanatory sequential design, with the quantitative phase taking precedence. We conducted an online survey of 235 healthcare workers at COVID-19 referral hospitals, followed by an interview with 11 participants from various hospital types.SettingCOVID-19 referral hospitals in Indonesia.ParticipantsHealth workers working at COVID-19 referral hospitals.MeasuresHospital ownership; hospital accreditation status; hospital readiness including incident management system, surge capacity, infection control and prevention, and human resource management; patient safety incident.ResultsAccording to the survey, 66.4% of the participants worked at a hospital owned by the provincial or district government, and 69.4% worked at a hospital which had received an excellent status accreditation. More than 80% of the hospitals scored well in the categories of the incident management system (86%), surge capacity (80.9%), infection control and prevention (97.9%), and human resource management (84.7%). However, only 50.6% of the hospitals scored well in managing patient safety incidents. Hospital ownership, accreditation status and hospital readiness all have an impact on patient safety incidents, which were reported in all types of hospitals by both studies.ConclusionsThis study provides significant results for Indonesia in terms of hospital preparedness and patient safety for the COVID-19 pandemic. The accreditation and ownership status of the hospital have aided hospital readiness. Despite the fact that no hospital in the world was prepared for the COVID-19 pandemic, hospital readiness has improved a year later; however, patient safety has not improved. Patient safety incidents occurred regardless of hospital status, with the most common occurrence being delayed treatment. Administrative errors were also recorded in COVID-19 field hospitals that were not accredited. Future research should focus on improving pandemic care quality and implementing initiatives that are applicable to all types of hospitals.

COVID-19 has been infecting every continent and Indonesia had suffered greatly as it ranked first for confirmed cases in Southeast Asia region with almost 100,000 deaths. We summarized Indonesian population demographic, socio-behavior, multiple government policy and public health interventions contributed to the high mortality. Although the virulence of COVID-19 strain was found to be similar as other countries, it might be inaccurate due to the low amount of sequenced genome and publicly accessible data of the virus’ strain. As Indonesia recorded higher testing number than WHO’s target, there were imbalances of testing capacity between capital cities and remote areas, hence the actual case number would be larger. The availability of healthcare facilities and skilled healthcare workers were also dispersed unequally, causing Indonesian health systems near collapsing. Moreover, individual sense of urgency and hazard of the pandemic were low, as shown by the low compliance of wearing masks and social distancing. Even though Indonesia was unprepared early in the pandemic, currently these systems are being improved. Indonesia has experienced an extensive problem in conquering the pandemic, hence, the country needed a comprehensive approach from the government, citizens, even private sectors to cease the pandemic.

Terpuruknya kegiatan ekonomi dirasakan pengusaha, karyawan dan wirausaha. Salah satunya adalah pabrik garmen di wilayah Bantul yang gulung tikar sehingga banyak karyawan pabrik di PHK secara sepihak. Program pengabdian masyarakat diberikan untuk menggerakan roda ekonomi masyarakat terutama bagi perempuan. Melihat kondisi tersebut kami tergerak untuk melatih sebagian karyawan garmen tersebut agar tetap berkarya memproduksi Alat Pelindung Diri (APD) seperti coverall medis dan gown baik dari bahan spunbond, parasut waterproof, taslan, dan microphorus. Pada awal pandemi COVID-19 antara kebutuhan APD dengan jumlah produksi tidak seimbang. Sehingga pemesanan APD yang masuk sangat banyak, permintaan dari Jawa, Sumatera, Kalimantan, Sulawesi bahkan sampai Papua. Kegiatan ini mampu memproduksi 5000 gown maupun coverall dalam 1 bulan. Kelompok kami berjumlah 10 penjahit yang beralamat di Randubelang, Semail dan Bangunjiwo Bantul. Pengabdian masyarakat ini dilakukan dengan metode pelatihan pembuatan APD, pembagian kerja dan membangun jaringan market. Pada proses pelatihan meliputi cara pembuatan desain APD, cara menjahit yang benar sesuai syarat sebagai alat pelindung diri, proses pengemasan dan pengiriman paket yang efektif dan murah. Hasil yang dirasakan oleh para penjahit adalah adanya pemasukan finansial yang lebih banyak dibandingkan saat mereka bekerja di pabrik. Banyak waktu untuk mengurus keluarga karena produksi dikerjakaan di rumah masing-masing, dan peluang resiko tertular virus COVID-19 lebih kecil. Diharapkan tetap berkarya dan produktif pada masa pandemi covid-19. Situasi ini dibutuhkan kerjasama yang terkoordinasi baik antar masyarakat dan pemerintah, sehingga UMKM tetap bertahan demi kelangsungan produksi. Perlu kemampuan beradaptasi dengan cepat, tepat dan benar untuk bertahan hidup baik terutama dari segi ekonomi.
