Simultaneous transcatheter pulmonary and tricuspid valve‐in‐valve implantation for the treatment of failing pulmonary and tricuspid bioprostheses

Abstract: We report a case of a patient with a history of bioprosthetic pulmonary and tricuspid valve replacement who developed significant stenosis and regurgitation of the bioprosthetic valves. Due to multiple comorbidities, he was deemed high risk for redo-valve replacement surgery. He subsequently underwent percutaneous transcatheter pulmonary and tricuspid valve-in-valve procedure at the same setting. This is the first of such that is reported in the literature. © 2015 Wiley Periodicals, Inc.

“…Among all, one patient received two valves in the same procedure – that is, in the pulmonary and tricuspid positions. Until now, there has been only one publication of a double implant of Sapien XT valve in a patient during the same procedure 7 . Our patient is the first young adult patient with CHD in Europe to undergo a double implant in the same procedure.…”

“…Schlomicher et al reported a simultaneous ViV procedure and TAVI for failed mitral and aortic positions with an uneventful postoperative course. Another case report by Baig et al reported a simultaneous transcatheter pulmonary and tricuspid ViV implantation for failed pulmonary and tricuspid bioprostheses in a 54‐year‐old man with multiple comorbidities.…”

“…The use of bioprosthetic valves has been on the rise the world over . However, failed surgical bioprostheses, particularly in terms of durability in younger individuals, have always been a concern for cardiologists and cardiac surgeons .…”

“…5 The risks of mortality and major morbidity are higher in the setting of redo sternotomy. 1,5 Combined transcatheter tricuspid valve replacement and pulmonary valve replacement (TTVR and TPVR, respectively) has been reported in patients with carcinoid heart disease 6,7 and congenital heart disease 8,9 whose cases were deemed to be high risk for operative intervention. Published data on combined TTVR/TPVR thus far have been limited to single case reports.…”
