Simultaneous sizing and preoperative risk stratification for thoracic endovascular aneurysm repair: Role of gated computed tomography

Abstract: This study introduces the feasibility of dynamic imaging of the thoracic aorta and cardiac structures and function, including the coronary arteries, with just one CT scan. The images could be successfully assessed for thoracic aorta pathology, cardiac disease, and extracardiac pathology. With further developments of CT scanners-and more detailed insight in the prognosis of patients based on ECG-gated CTA findings-this new technique may become the initial imaging modality for preoperative cardiac risk stratific… Show more

“…1,64 The latest innovations in clinical practice include electrocardiographically gated 66 aortic computed tomographic studies leading to high-quality, precise imaging of the ascending aorta, as well as simultaneous evaluation of the coronary arteries 67 ( Figure 15). Electrocardiographically gated CTA adds valuable information in the study of aortic pathology involving the aortic root and valve, 68 in congenital heart disease, 69 for simultaneous aortocoronary evaluation, 66 for planning of endovascular therapy, 68,70 for imaging of the postsurgical ascending aorta, 71 and to show dynamic changes of true luminal compression in aortic dissection. 72 The main drawbacks of CTare the use of ionizing radiation and iodinated contrast media (ICM).…”

“…In addition, motion artifact can adversely affect the resolution of CT images of the aorta, producing changes in diameter as great as 7.5% to 27.5%, although technology is improving, especially with gated tomographic angiography (1)(2)(3).…”
