AimTo analyse the effect of leucocyte‐ and platelet‐rich fibrin (L‐PRF) on bone regeneration procedures and osseointegration.Materials and MethodsAn electronic and hand search was conducted in three databases (MEDLINE, EMBASE and Cochrane). Only randomized clinical trials, written in English where L‐PRF was applied in bone regeneration and implant procedures, were selected. No follow‐up restrictions were applied.ResultsA total of 14 articles were included and processed. Three subgroups were created depending on the application: sinus floor elevation (SFE), alveolar ridge preservation and implant therapy. In SFE, for a lateral window as well as for the trans‐alveolar technique, histologically faster bone healing was reported when L‐PRF was added to most common xenografts. L‐PRF alone improved the preservation of the alveolar width, resulting in less buccal bone resorption compared to natural healing. In implant therapy, better implant stability over time and less marginal bone loss were observed when L‐PRF was applied. Meta‐analyses could not be performed due to the heterogeneity of the data.ConclusionsDespite the lack of strong evidence found in this systematic review, L‐PRF might have a positive effect on bone regeneration and osseointegration.

Résumé -En odontologie, l'indication de la pose d'un implant peut être limitée par le volume du tissu osseux alvéolaire résiduel. La réalisation d'une greffe osseuse ou l'apport d'un matériau de substitution est parfois néces-saire. Le PRF (platelet-rich fibrin) est un concentré plaquettaire autogène utilisé pour stimuler la néoformation osseuse et favoriser la cicatrisation. C'est une préparation d'utilisation récente. L'intérêt de ce travail repose sur la gestion des tissus durs et des tissus mous en chirurgie pré-implantaire. Il se donne comme objectif de montrer l'intérêt clinique d'une technique de préparation simple en vue d'une régénération tissulaire en chirurgie orale, plus particulièrement en implantologie dentaire. Cette étude décrit la préparation du PRF et son utilisation. Il s'agit d'une préparation extemporanée, réalisée au « bloc opératoire odontologique » sur des patients en vue d'une pose d'implant. Du sang veineux est prélevé, mis dans des tubes sans anticoagulant et passé à la centrifugeuse à 3000 tours/mn pendant 10 mn pour obtenir un gel de PRF. Ce gel est pressé entre deux compresses stériles pour obtenir une membrane de PRF. Il est utilisé seul ou mélangé à de l'os autogène ou à un matériau de substitution et placé sous le lambeau, contre le tissu osseux résiduel. À Madagascar, l'implantologie dentaire s'est déve-loppée seulement depuis quelques années, et l'apport du PRF apparaît très prometteur. Toutefois, il doit être utilisé avec prudence car la littérature n'est pas unanime sur ses indications.Abstract -The PRF in dental implant: a promising technique in Madagascar.
