Simultaneous Noncentered Photoactivated Chromophore for Keratitis-Corneal Collagen Cross-Linking and Penetrating Keratoplasty for Treatment of Severe Marginal Fusarium spp. Keratitis: A Description of a New Surgical Technique

Abstract: The purpose of this article is to describe the use of simultaneous noncentered photoactivated chromophore for keratitis-corneal collagen cross-linking (PACK-CXL) combined with penetrating keratoplasty in the treatment of a severe marginal Fusarium spp. keratitis case with imminent corneal perforation. It is a retrospective case report study; it was performed by collecting clinical data, images, video, and postoperative evaluations. The clinical control of the infection was accomplished, despite difficulties in… Show more

“…This treatment continues to be the gold standard for this pathology. The CXL-PACK technique was first described by Spörl in 1997, but the antimicrobial properties of riboflavin in combination with UV light have been known since 1965 [ 7 ]. In this patient, the Dresden protocol for CXL-PACK was performed [ 8 ].…”

“…Cross-linking treatment has recently emerged as a therapeutic option previous to performing keratoplasty. Numerous studies demonstrate the efficacy of ultraviolet light for the inactivation of microorganisms [ 5 , 6 , 7 , 8 ].…”

“…Conversely to ours in that case, the abscess was limited to the 3–5 o’clock area and was not associated with corneal melting and graft failure. In a recent case report, Balparda et al [12] proposed the application of PACK-CXL combined with PKP for the treatment of severe marginal Fusarium spp. keratitis on a virgin cornea.…”
