Simultaneous coverage of two separate dorsal digital defects with a syndactylizing venous free flap

Abstract: We report a case of a 58-year-old patient who sustained multiple finger injuries on the right hand. After thorough debridement, two dorsal defects of the middle and ring fingers were covered simultaneously with a single arterialized venous free flap from the right forearm. The flap was used to create a dorsally syndactylized digit which survived completely and was subsequently divided longitudinally. With early flap division, excellent functional and cosmetic result was achieved. We consider the syndactylized … Show more

“…32 In our practice, the AVF was also used to reconstruct two separate dorsal digital defects with a syndactylizing venous free flap simultaneously and achieved excellent functional and cosmetic result. 46 Avulsion injuries of the hand or degloving injuries of fingers are some of the most intractable clinical scenarios. This flap was applied to salvage the avulsed palmar tissue with good results.…”

“…The development of microsurgery has greatly boosted the application of various free flaps and venous flaps for the repair of finger wounds, especially when pedicle flaps are unable to cover the large defect . Although the dorsalis pedis flap has been reported to resurface these wounds, producing large free flaps that are suitable for a single large wound on the hand . These free flaps are not the favorable options for coverage of multifinger soft tissue defect.…”

“…These free flaps are not the favorable options for coverage of multifinger soft tissue defect. They could only achieve a dorsally syndactylized digit after the primary procedure and required several division operations, each of which would involve high risks of flap necrosis …”

“…This technique provided an aesthetically pleasing result without syndactylizing the digits with a pedicled or free flap, or by using multiple local flaps, which would have left more scars on the injured hand (Borschel, 2009;Hyza et al, 2005;Karacaoglu and Gokce, 2008;Trovato et al, 2008;Ulrich and Pallua, 2009). …”
