Simultane hyperfraktionierte, akzelerierte Radiotherapie mit 5-FU und einmal wöchentlich Cisplatin bei lokal fortgeschrittenem Kopf-Hals-Tumor

Abstract: Chemoradiation with 5-FU and cisplatin seems feasible and superior in terms of LRC and OS to the ARO 95-06C-HART arm at 2 years. However, this did not persist at the 5- and 10-year follow-ups.

“…In our opinion, squamous cell carcinomas in this area mostly represent metastatic spread rather than primary salivary gland disease. They behave essentially like other squamous cell carcinomas of the head and neck area and should therefore be treated accordingly (especially with regard to the indications for concomitant chemotherapy) . Local therapeutic options, including surgery, which need to be followed by radiotherapy in high‐risk cases are the mainstay of therapy for salivary gland cancer.…”

“…Two different chemotherapy dose schedules were used: 100 mg/m 2 body surface area every 3 weeks, or weekly doses of 30 mg/m 2 body surface area for 6 to 7 cycles. This low dose was selected based on previous experience with similar patients, having observed that many do not tolerate the high‐dose schedule, despite being treated on an inpatient basis and having access to optimal supportive care . During the treatment planning process, optimal sparing of normal tissue was mandatory.…”

Wherever possible, the parotid glands were spared during radiotherapy.…”

“…The prognosis of HNSCC patients with locoregionally advanced tumors is poor as indicated by five year overall survival rates of 40-60% in recent clinical trials [26]. In recurrent and metastatic disease the mean overall survival does not exceed 11 months despite intensive treatment protocols [7, 8].…”
