PurposeIn obstetrical emergency situations, optimal management requires the immediate coordinated actions of a multi-disciplinary and multi-professional team. This study investigated the influence of simulation training on four specific skills: self-confidence, handling of emergency situation, knowledge of algorithms and team communication.MethodsClinical algorithms were first presented to the participants. Training for six emergency situations (shoulder dystocia, postpartum haemorrhage, pre-eclampsia, maternal basic life support, neonatal resuscitation and operative vaginal birth) was performed using high- and low-fidelity simulation mannequins. General impression of the simulation training and the four above-mentioned skills were evaluated anonymously through a self-assessment questionnaire with a five-point Likert scale immediately after the training and 3 months later.ResultsFrom November 2010 to March 2012, 168 participants, distributed over six one-day courses, took part in the training. 156 participants returned the questionnaire directly after the course (92.9 %). The questionnaire return rate after 3 months was 36.3 %. The participants gave higher Likert scale answers for the questions on the four specific skills after 3 months compared to immediately after the course. The improvement was statistically significant (p ≤ 0.05) except for the question regarding team communication.ConclusionImplementation of simulation training strengthens the professional competency.

Severe pre-eclampsia and hemorrhagic postpartum were main causes of maternal death in Indonesia which could not handling appropriately by midwife as health worker. One of effort to decrease maternal death cause by emergency obstetric is a training skill by simulation to improving midwife responsive skill when handling emergency obstetric cases. The simulation of activities are reduces stress for the midwife as well as risk of injury and discomfort to the client. Thus, effective use of models (humanistic approach) is an important factor in improving the quality of clinical skills. Providing quality emergency obstetric simulation on training reduces the risk of maternal mortality and morbidity. The objective was to review the relation of simulation with midwife handling emergency obstetric case successfully. This study used cross-sectional method with midwife of total sampling. The result view that simulation of handling emergency obstetric case taken by midwife average 2,3 times with 93% when handling case of severe preeclampsia and 98,2% postpartum handling. Then the bivariate analysis using Chi square test obtained there was significant relationship between simulation of frequent with midwife handling emergency obstetric case successfully with ρ value = 0,042 for handling emergency of severe pre-eclampsia and ρ value = 0,034 for hemorrhagic postpartum handling successfully. Conclusion: There are significant result between midwife skills simulation with handling emergency obstetric case successfully.

A Surgical Simulation Curriculum for Senior Medical Students Based on TeamSTEPPS

Setting and Intervention: We integrated a TeamSTEPPS module into our existing resident readiness elective. The curriculum included interactive didactic sessions, discussion groups, role-plays, and videotaped immersive simulation scenarios. Main Outcome Measures: Improvement of selfassessment scores, multiple-choice examination scores, and performance ratings of videotaped simulation scenarios before and after intervention. The videos were rated by masked reviewers on the basis of a global rating instrument (TeamSTEPPS) and a more detailed nontechnical skills evaluation tool (NOTECHS). Participants: Seventeen students participated and completed the study. Results: The self-evaluation scores improved from 12.76 to 16.06 (P Ͻ.001). The increase was significant for all of the TeamSTEPPS competencies and highest for leadership skills (from 2.2 to 3.2; PϽ .001). The multiplechoice score rose from 84.9% to 94.1% (PϽ.01). The postintervention video ratings were significantly higher for both instruments (TeamSTEPPS, from 2.99 to 3.56; P Ͻ.01; and NOTECHS, from 4.07 to 4.59; PϽ.001). Conclusions: The curriculum led to improved selfevaluation and multiple-choice scores as well as improved team skills during simulated immersive patient encounters. The TeamSTEPPS framework may be suitable for teaching medical students teamwork concepts and improving their competencies. Larger studies using this framework should be considered to further evaluate the generalizability of our results and the effectiveness of TeamSTEPPS for medical students.
