Simulation for High-Stakes Evaluation in Nursing

Abstract: Simulation is prevalent in nursing education yet is rarely used for evaluation. The authors describe the use of simulation as a high-stakes evaluation of senior baccalaureate nursing student achievement of end-of-program competencies and assist with student remediation. The results of a high-stakes evaluation led to positive changes in student learning as well as curriculum reform for a Midwestern university.

“…Simulation allows learners to practice and demonstrate competence in areas involving knowledge, skills, critical thinking, and communication. 8 There are many types, ranging from low-fidelity simulation to high-fidelity simulation. It can use mannequins or people to create a learning environment that mimics clinical practice.…”

“…Simulation-based medical education (SBME) is a powerful educational strategy enabling students to practice and demonstrate competence in areas including knowledge, skills, critical thinking and communication thereby enriching learning experience (Motola et al 2013;Salas et al 2013;Bensfield et al 2012;Issenberg et al 2005). It provides a bridge between didactic and observational learning to clinical practice using experiential learning, however it has rarely been used to teach medical students about ethical conflicts or to assess their understanding of ethical principles applied to difficult clinical decision making (Tritrakarn 2014).…”

“…Nearly 25% of the students did not pass the summative assessment and had to participate in remediation. The process, however, revealed gaps in the curriculum as well as the need for students to be comfortable with simulation for instructional purposes before it is used for high-stakes assessment (Bensfield et al, 2012).…”

“…Bensfield, Olech, and Horsley (2012) used simulation-based assessment of senior baccalaureate nursing students to determine whether they achieved program competencies and to guide remediation. The objectives for the evaluation were derived from the program outcomes and four quality and safety competencies (patient-centered care, teamwork and collaboration, evidence-based practice, and safety).…”
