Simplified sperm retrieval and intracytoplasmic sperm injection in patients with azoospermia

Abstract: This extensive series of PESA/ICSI cycles indicates that PESA can be used successfully to retrieve spermatozoa in patients with azoospermia. The technique is simple, cost-effective and is associated with fewer complications compared to an open microsurgical procedure.

“…When making a decision as to what to do, one must also take into account that epididymovasostomy carried out in the most effi cient manner, by microsurgery, is a difficult procedure requiring considerable microsurgical skill, and the out come of successful fertility, as in this small series, is not good [14,15]. The situation has altered, however, in the last year or so, with the introduction of direct sperm aspi ration from the epididymis or testis and intracytoplasmic sperm injection (ICSI) into the ovum, with successful out come in many cases [16]. Fertilisation is therefore achiev able in patients in whom vasovasostomy has failed, and the alternatives should be fully discussed with the patient before proceeding with a second attempt.…”

“…IN THE TREATMENT of severe oligozoospermia or azoospermia, a number of techniques including microsurgical epididymal sperm aspiration (MESA) 7,8 or percutaneous epididymal sperm aspiration (PESA) 8,9 have been used. It is the advantage of TESE compared with such procedures to provide a histological evaluation of the testicular biopsy in parallel with it 10 …”
