Background: Acute kidney injury (AKI) during hospitalisation is frequent and associated with adverse outcomes.Aims: To evaluate the association between renal function recovery after AKI and short-term post-discharge mortality.Methods: This is a retrospective study of all AKI episodes codified in the electronic records of a single centre in 2013 and 2014. Epidemiological data and comorbidities at baseline and laboratory values at admission and discharge were collected. Persistent kidney dysfunction after AKI was defined as a last serum creatinine equal or above 1.2-fold over baseline level. Patients were followed for 30 days after discharge.Results: Out of 1720 evaluated patients, 1541 (89%) were analysed. Of them, 869 (56%) recovered renal function. Independent predictors of renal function recovery after AKI were lower baseline estimated glomerular filtration rate (eGFR) (P < 0.001), higher admission eGFR (P < 0.001) and haemoglobin (P = 0.016), milder AKI (P = 0.037), absence of a history of heart failure (P < 0.001) and lower admission blood pressure (P < 0.001). After discharge, 46 (3%) patients died in the first 30 days. Persistent kidney dysfunction was associated (P = 0.01) with and independently predicted (odds ratio 2.6; 95% confidence interval 1.2-5.4; P = 0.01) short-term post-discharge mortality.Conclusions: Persistent kidney dysfunction after an AKI episode is an independent predictor of 30-day post-discharge mortality. This information might help select AKI patients who require closer follow up and monitoring after discharge.

Late-life cognitive decline ranges from the mildest cases of normal, age-related change to mild cognitive impairment to severe cases of dementia. Dementia is the largest global burden for the 21st century welfare and healthcare systems. The aim of this study was to analyze the neuropsychological constructs (temporal orientation (TO), spatial orientation (SO), fixation memory (FM), attention (A), calculation (C), short-term memory (STM), language (L), and praxis (P)), semantic fluency, level of functionality, and mood that reveal the greatest deficit in the different stages ranging from normal cognition (NC) to cognitive impairment in older adults in a primary healthcare setting. The study included 337 participants (102 men, 235 women), having a mean age of 74 ± 6 years. According to their scores on the Spanish version of the Mini-Mental State Examination (MEC-35), subjects were divided into 4 groups: no deterioration (ND) (score 32–35), subtle cognitive impairment (SCI) (score 28–31), level deterioration (LD) (score 24–27) and moderate deterioration (MD) (score 20–23). The ND group revealed significant differences in TO, STM, C, A, L, P, and S-T as compared to the other groups. The MD group (in all the neuropsychological constructs) and the ND and SCI groups showed significant differences on the Yesavage geriatric depression scale (GDS-15). All except the FM neuropsychological construct were part of the MEC-35 prediction model and all of the regression coefficients were significant for these variables in the model. Furthermore, the highest average percentage of relative deterioration occurs between LD and MD and the greatest deterioration is observed in the STM for all groups, including A and TO for the LD and MD groups. Based on our findings, community programs have been implemented that use cognitive stimulation to prevent cognitive decline and to maintain the neuropsychological constructs.

Fundamento. El desarrollo de un fracaso renal agudo (FRA, definido siguiendo las guías KDIGO) durante una hospitalización en pacientes nonagenarios ha sido poco estudiado. El objetivo del presente estudio es analizar la mortalidad en pacientes nonagenarios hospitalizados que desarrollan un FRA.Métodos. Se recogieron todos los pacientes nonagenarios que desarrollaron FRA durante el ingreso hospitalario entre 2013 y 2014. Basalmente, se recogieron variables epidemiológicas, comorbilidades y, durante el ingreso, variables analíticas y mortalidad. Se analizaron las variables asociadas a mortalidad durante el episodio de FRA y los predictores independientes mediante regresión logística.Resultados. Se incluyeron 264 pacientes nonagenarios con FRA. La edad media fue 93±3 años, siendo 73 (27,7 %) varones. Durante la hospitalización fallecieron 79 pacientes (29,9 %). Las comorbilidades asociadas a mortalidad fueron la insuficiencia cardiaca (p = 0,018), la disfunción diastólica (p = 0,042) y un mayor nivel o grado de dependencia (p = 0,003). Las variables clínicas en el momento del ingreso que se asociaron a mortalidad fueron presión arterial sistólica y diastólica más baja (p = 0,016 y 0,013, respectivamente), leucocitosis (p = 0,003), mayor severidad del FRA valorado por AKIN (p = 0,003) y valor de ácido L-láctico más alto (p = 0,005). Los predictores independientes de mortalidad fueron la insuficiencia cardiaca (OR = 2,31; IC95%: 1,07-5,00; p = 0,036), la dependencia valorada por el índice de Barthel modificado (OR = 0,80; IC95%: 0,67-0,97; p < 0,016) y el L-láctico al ingreso (OR = 1,31; IC95%: 1,06-1,61; p = 0,005).Conclusión. La insuficiencia cardiaca, el nivel de dependencia y el ácido L-láctico al ingreso son predictores independientes de mortalidad en pacientes nonagenarios hospitalizados con FRA.
