Simple ultrasound rules to distinguish between benign and malignant adnexal masses before surgery: prospective validation by IOTA group

Abstract: Objectives To prospectively assess the diagnostic performance of simple ultrasound rules to predict benignity/malignancy in an adnexal mass and to test the performance of the risk of malignancy index, two logistic regression models, and subjective assessment of ultrasonic findings by an experienced ultrasound examiner in adnexal masses for which the simple rules yield an inconclusive result. Design Prospective temporal and external validation of simple ultrasound rules to distinguish benign from malignant adne… Show more

“…5 can be classified using subjective assessment by an experienced ultrasound operator, or given the high prevalence of malignancy in this group they can all be classified as malignant to increase the sensitivity for ovarian cancer. 6 On prospective validation both by the IOTA group (two studies including 1938 and 2403 patients, respectively) 7,8 and by other research teams (nine studies including a total of 2101 tumors) 9-17 , the Simple Rules were applicable in 77 to 94% of tumors (range between studies). The malignancy rate ranged from 1% to 9% in cases classified as benign, from 69% to 94% in cases classified as malignant, and from 13% to 53% in inconclusive cases.…”

“…Still, many patients with preoperative RMI above 200 had their primary surgery in RH. New and improved (13). However, if gynecologists at RH do not follow the guidelines and do not refer the patient to a TC when ovarian malignancy is suspected, there is little use for these algorithms and the patients are at risk of not receiving optimal treatment.…”

“…Swollen left iliac lymph nodes were recognized but without clinical significance. Based on the above reported ultrasonographic data and regarding IOTA simple rules [7,8], the ovarian lesion was characterized as a "probably malignant tumor" (Figure 1). …”
