Simple Repair of a Blow-Out Fracture by the Modified Caldwell–Luc Approach

Abstract: Here we report a patient with a blow-out fracture of the orbital floor that was treated by an intraoral transmaxillary approach. This 38-year-old man suffered a sudden blow to the periorbital area, which caused prolapse of the orbital contents into the maxillary sinus. The modified Caldwell-Luc approach was used to repair the orbital blow-out fracture and the maxillary sinus during was packed with Frazin gauze for 7 days to prevent recurrence of the prolapse. This was an easy and minimally invasive technique f… Show more

“…Three main approaches can be used for the consolidated management of OMS: an intraoral approach through the originating tooth site, endoscopic approach through the nose and OMU, and intended upper maxilla approach after making bony window. Recently, modified endoscopy-assisted maxillary sinus surgery (MESS) was applied for the intraoral reduction of blowout orbital fractures [65], removal of sinus pathologies [66], and removal of migrated implants beneath the optic canal [67]. MESS is a new, innovative sinus-approach surgical procedure, which is efficient, easy, and less complicated than other sinus approaches (Fig.…”

“…Versatile applications for the MESS technique have been reported in recent literature, such as the intraoral reduction of blow-out orbital fractures [11] and the treatment of mucous retention cysts or maxillary sinusitis [12]. The main advantage of this renovated procedure is that the physiological functions of the paranasal sinus (PNS) are maintained while avoiding formation of POMC or sinus scar tissues.…”

“…7 The most common radiographic characteristic of FD on CT is an expanded bone with ground-glass appearance. [1][2][3][4][5][6][7][8] However, on CT, FD is often seen as a sclerotic and lytic form or a cystic change rather than a homogeneous ground-glass form. 4 Furthermore, on MRI scans, FD can be highly variable, sometimes having the appearance of an aggressive neoplasm.…”

“…However, prophylactic optic nerve decompression could not be recom-mended. 1,2,8 Therefore, in this study, only 1 patient complaining of blurred vision underwent optic nerve decompression, and the remaining 14 patients with FD of the sphenoid sinus were followed up without surgery because there was no change in eye symptoms.…”
