Simple Coiling versus Stent-Assisted Coiling of Paraclinoid Aneurysms : Radiological Outcome in a Single Center Study

Abstract: ObjectiveParaclinoid aneurysms are a group of aneurysms arising at the distal internal carotid artery. Due to a high incidence of small, wide-necked aneurysms in this zone, it is often challenging to achieve complete occlusion when solely using detachable coils, thus stent placement is often required. In the present study, we aimed to investigate the effect of stent placement in endovascular treatment of paraclinoid aneurysms.MethodsData of 98 paraclinoid aneurysms treated by endovascular approach in our cente… Show more

“…Stent-assisted coiling is commonly used in the treatment of unruptured, wide-necked aneurysms. The deployment of a stent prevents the coil from protruding into the parent artery and may thus reduce the recurrence rate ( 16 , 17 ). However, the deployment of a stent in the acute stage may incur additional complications, such as in-stent thrombosis ( 7 , 18 , 19 ).…”

“…Previous reports of LPAs only involved treatment studies of all paraclinoid aneurysms 4 , 13 , 23) or small surgical case series. 12) A key finding of the present study was that ICA angle is a hemodynamic factor that contributes to aneurysm morphology.…”

“…However, there are only a few reports of aneurysms with a dome projecting laterally and the neck originating from the lateral wall of the paraclinoid potion of the internal carotid artery (ICA). 4 , 12 , 13) These aneurysms are of the sidewall-type, occur at non-branching sites that do not have perforating vessels, and have an orifice located on the lesser curve side of the ICA bend at the level of the anterior clinoid process. The level of hemodynamic stress on the aneurysm can change depending on the degree of lateral flexion of the ICA as the parent artery, which may have a hemodynamic effect on aneurysm morphology and occlusion status after endovascular treatment.…”

“…The deployment of stent can protect coil from protruding into parent artery. Additionally, it can reduce the recurrence rate [16][17]. However, the deployment of stent in acute stage may incur more complications such as instent thrombosis [7,18,19].…”
