Abstract: A 30‐year‐old woman presented with low‐grade dyspnea on exertion. Chest X‐ray demonstrated enlarged cardiac silhouette but was insufficient to delineate the cause. Echocardiogram revealed the cause to be the giant left atrium from mitral stenosis.

“…fected due to the active remodeling of the critically small LV cavity towards a certain increase in its size already at the hospital stage [8,9,10,11,12,13,14,15]. The conjugal process does not allow to quickly increase the volume of LV, so the size of the mitral mechanical prosthesis (25 mm) is adequate in this clinical situation.…”

“…Left atriomegaly in the presence of AF tachyform contributes to the occurrence of blood clots in the cavity of the LA and thromboembolic complications, therefore, during correction, it is necessary to make a maximum decrease in the volume of LA. In addition, dilated LA contributes to the progression of cardiorespiratory failure due to compression of the bronchi, trachea and posterior wall of the LV [5,7,8,9,10,11].…”

“…In Marfan syndrome and mitral valve prosthetics, complete preservation of valvular and subvalvular structures can significantly reduce the critically increased volume of LV, maintain its contractility, especially in the presence of ventriculomegaly, which gives manifestations of critical HF [8,9,10,11]. Functional recovery of the heart (decrease in the level of latent HF) is observed only by 4 months, and only thanks to powerful therapy (Uperio, prednisone), which is an urgent element of the treatment strategy, starting from the preoperative stage.…”
