Sildenafil Improves the Beneficial Haemodynamic Effects of Intravenous Nitrite Infusion during Acute Pulmonary Embolism

Abstract: Acute pulmonary embolism produces acute pulmonary hypertension, which can be counteracted by activating the nitric oxide-cyclic guanosine 3 ′ ,5 ′ -monophosphate (cGMP) pathway. While previous studies have shown that sildenafil (an inhibitor of cGMP-specific phosphodiesterase type 5) or nitrite (a storage molecule for nitric oxide) produces beneficial effects during acute pulmonary embolism, no previous study has examined whether the combination of these drugs can produce additive effects. Here, we expand prev… Show more

“…Plasma lipid peroxide levels were determined by measuring thiobarbituric acid-reactive substances using a fluorimetric method as previously described (Dias-Junior et al 2008). This method requires excitation at 515 nm and emission at 553 nm and uses 1,1,3,3-tetramethoxypropane as standard.…”

“…Notably, from a therapeutic standpoint, short-term delivery of nitrite dissolved in saline through nebulization produced selective, sustained pulmonary vasodilation with no clinically significant increase in blood methemoglobin levels [41]. Experimental evidence also indicates that nitrite therapy for the treatment of pulmonary hypertension may not be limited to the newborn and may not be limited to an inhaled route as there is data to suggest that intravenous nitrite therapy can reduce pulmonary hypertension in an adult model of acute pulmonary thromboembolism [66]. These data support the concept that nitrite is a vasodilator acting through conversion to NO and support the notion that it may be a potential therapy for pulmonary hypertension.…”

“…Several case reports on successful use of sildenafil in this setting have been reported and Dias-Junior et al demonstrated in a dog model that sildenafil effectively lowers the pulmonary pressure after acute PE [8][9][10][19][20][21][22][23][24]. Based on these reports we have used sildenafil in the reported and recently in another patient (Table 1).…”
