Significant correlations of plasma homocysteine and serum methylmalonic acid with movement and cognitive performance in elderly subjects but no improvement from short-term vitamin therapy: a placebo-controlled randomized study

Abstract: High plasma tHcy and serum MMA were prevalent and correlated inversely with movement and cognitive performance. Oral B vitamin treatment normalized plasma tHcy and serum MMA concentrations but did not affect movement or cognitive performance. This might have been due to irreversible or vitamin-independent neurocognitive decline or to an insufficient dose or duration of vitamins.

“…Many of these trials however were of insufficient power and duration to detect an effect or included participants with existing optimal B-vitamin status or with advanced dementia, where a beneficial effect is not likely (60,62,(67)(68)(69)(70) . Notably, two similarly designed homocysteine-lowering trials have yielded conflicting results.…”

“…The factors that contribute to the malabsorption of food cobalamin include chronic presence of Helicobacter pylori, achlorhydria and intestinal microbial proliferation, which can be brought on by antibiotic treatment (3,5) . In patients with pernicious anaemia, studies of methylmalonic acid (MMA) and homocysteine values compared with vitamin B 12 levels have determined that metabolites are elevated prior to the development of any clinical abnormalities and often prior to abnormal serum vitamin B 12 concentrations (6) . A correct diagnosis of vitamin B 12 deficiency is complicated due to the poor sensitivity and specificity of conventional kit assays (7) .…”
