Signalment, clinical signs, and prognostic indicators associated with high-grade second- or third-degree atrioventricular block in dogs: 124 cases (January 1, 1997–December 31, 1997)

Abstract: Pacemaker implantation should be strongly considered in all dogs with high-grade second- or third-degree AVB regardless of whether clinical signs are evident. If medical treatment is warranted, vagolytic medications may be the best choice. A new classification system for AVB may merit further investigation.

“…1,2 There are numerous reports of AV block associated with myocardial lymphoma in humans, where infiltrative neoplasia may obliterate the normal AV nodal tissue. 1,3 In contrast to our case, when infiltrative myocardial lymphoma is diagnosed in human beings, the majority are of B cell origin.…”

“…4 Since that time, various technologic advances adapted from human medicine have allowed the safe implantation of more sophisticated devices in dogs resulting in improved survival time and high rates of owner satisfaction with a relatively low complication rate. 3,5,6 Single chamber AP systems have traditionally been implanted in veterinary medicine, and the technique has been well described. 7 Single lead transvenous endocardial pacing with the lead positioned at the right ventricular apex is most commonly used.…”

“…10 Treatment of dogs with complete AVB includes a thorough attempt to elucidate and address the underlying cause. 2,9 Pacemaker implantation is generally required to resolve clinical signs and provides a more favorable prognosis for duration of survival. 1,11 However, the etiology of third-degree AVB is uncommonly elucidated antemortem.…”
