Should we test hepatitis C virus genotype and viraemia level in patients with chronic hepatitis C?

Abstract: Hepatitis C is a major health problem and at present, interferon-alpha is the only therapy of proven use. Two viral factors, namely HCV genotype and viraemia levels, have been studied extensively for their possible role in the clinical management of patients with chronic hepatitis C virus infection. This review attempts to summarize our current understanding of these two viral factors and provide recommendations as to how these two parameters should be used by general practitioners, practising gastroenterologi… Show more

“…Some HCV genotypes, particularly 1b, are considered to be more virulent and more resistant to treatment by interferon-␣ [Dusheiko and Simmonds, 1994b;Dusheiko et al, 1996;Bruno et al, 1997;Isaacson et al, 1997;Kiyosawa, 1997;Mihm et al, 1997;Pawlotsky et al, 1998]. Continued surveillance will monitor spread of possibly more virulent and drugresistant HCV genotypes.…”

“…Different genotypes may carry different clinical significance Bruno et al, 1997;Kiyosawa, 1997;Mihm et al, 1997]. They may also influence response to interferon-alpha, currently the only therapeutic drug licensed for the treatment of HCV infection Dusheiko et al, 1996;Isaacson et al, 1997;Kiyosawa, 1997].…”

“…As serum HCV load is not per se a useful indicator in the selection of patient candidates for liver biopsy, we suggest that biopsy should be routinely performed in all subjects with PNAL (4, 6), as it remains the only means to delineate the presence and the degree of liver injury in these patients. Further long-term studies will be needed to evaluate the role of viremia in the management of patients with chronic HCV infection (59), and to establish whether the natural history of chronic HCV infection in "asymptomatic" HCV carriers with higher HCV RNA titer is different from those with a lower degree of viral replication.…”

“…These genotyping results can be useful in viral transmission studies as well as in epidemiological investigations. Furthermore, in many studies, associations between viral genotype, interferon responsiveness, the progression of disease, and the likelihood of developing hepatocellular carcinoma have been demonstrated (6,11,17,18,21,22,34,38,39). Thus, although the basis for these associations is poorly understood, HCV genotyping is being performed increasingly in clinical studies of HCV patients.…”
