Breast cancer-related lymphedema is a treatment-related chronic disease that causes great distress and medical burden. Early screening and precautionary measures for lymphedema could improve well-being and decrease medical costs. Herein, we used bioelectrical impedance analysis for early screening of lymphedema. We set up a verifiable standardized subclinical standard to screen subclinical lymphedema in postoperative breast cancer patients using bioelectrical impedance. The first part determined the criteria of subclinical lymphedema. Among the 424 female participants, 127 were healthy women, whereas 297 were postoperative breast cancer survivors. Subclinical standard boundaries were determined by the 95% confidence interval of the healthy women. The screening rate of patients with subclinical lymphedema was inferred by comparing the subclinical standard boundaries and the postoperative patient values. A total of 14.81–20.87% of postoperative breast cancer survivors were identified as patients with subclinical lymphedema. The second part provided the results of the verification test of this subclinical standard. The data of the verification test from 30 healthy women and 30 screened patients met the subclinical standard, and 30 breast cancer survivors with lymphedema verified the utility and feasibility of the subclinical standard. Therefore, this standard could provide a screening tool for early the identification of subclinical breast cancer survivors. Early detection helps implement personal and precise medical precautions for patients with subclinical lymphedema.

ÖZET Kardiyovasküler hastalıklar, kalp ve kan damarlarının patolojisi olup, dünya çapında primer ölüm nedenidir. Her yıl yaklaşık 17,9 milyon insanın kardiyovasküler hastalıklar nedeniyle yaşamını kaybettiği bilinmektedir. Kardiyovasküler hastalıkların büyük bir çoğunluğunun obezite, hipertansiyon, sedanter yaşam tarzı, sigara kullanımı gibi değiştirilebilir risk faktörlerine müdahale edilerek önlenebilmesi, erken teşhis ve tedavinin önemini artırmaktadır. Özellikle beslenme ve beslenme ile ilişkili faktörler kardiyovasküler hastalıkların hem önlenmesinde hem de tedavisinde kilit rol oynamaktadır. Adipozitenin artışı, inflamasyon ve oksidatif stresi artırarak kardiyovasküler hastalık gelişimine neden olmaktadır. Bu nedenle çeşitli otoriteler klinik uygulamada, adipozitenin değerlendirilmesinin kardiyovasküler hastalık riskinin belirlenebilmesi açısından gerekliliğini vurgulamaktadır. Antropometrik ölçümlerin ve laboratuvar yöntemlerinin bu amaçla kullanılması riskin değerlendirilmesinde prognostik bilgi sağlayabilir ve primer korumada önemli olabilir. Cardiorenal syndrome (CRS) is a complex disease in which the heart and kidney are simultaneously affected, and subsequently the malfunction of one organ promotes the deterioration of the other. Heart failure (HF) with preserved ejection fraction (HFpEF) is the most common form of HF. The pathophysiology of CRS is not well known and several mechanisms have been involved. An elevation of central venous pressure seems to be one of the key points to consider, among others such an increase of intraabdominal pressure. Several diagnostic tools have been identified to establish the diagnosis of CRS in patients with HFpEF. Currently, the availability of biomarkers of renal and cardiac injury, the use of pulmonary ultrasound (LUS), the monitoring of the size of the inferior vena cava and the study of the renal venous pattern offer a new dimension in accurately diagnosing and quantifying organ damage in CRS. Beyond the symptomatic treatment of congestion, until recently specific therapeutic tools for patients with CRS and HFpEF were not available. Interestingly, the development of new drugs such as the angiotensin/neprilysin inhibitors and sodium-glucose cotransporter-2 (SGLT-2) inhibitors offer new therapeutic strategies with potential benefits in reduction of cardio-renal adverse outcomes in this population. Randomized clinical trials focused on patients with HFpEF are currently ongoing to delineate optimal new treatments that may be able to modify their prognosis. In addition, multidisciplinary teamwork (nephrologist, cardiologist and nurse) is expected to decrease the number of visits and the rate of hospitalizations, with a subsequent patient benefit.
