Should We Label All Synovial Cysts as Unstable?

Abstract: Study Design:Retrospective study. Objective:To analyze the various anatomical parameters that influence segmental stability in patients suffering from lumbar intra spinal cysts (LISCs) and to determine the outcome of microscopic unilateral laminotomy and cystectomy. Methods:All patients that were surgically managed for a LISC between 2007 and 2013 with more than 3 years of follow-up were reviewed. Those without associated instability were evaluated for segmental mobility, segmental angulation, facet inclination,… Show more

“…Whether fusion is needed has also been discussed in patients with lumbar FJCs undergoing decompressive operation ( 26 ). The NeuroSpine Surgery Research Group proposed a classification system (grades 1–5) for lumbar FJCs based on the percentage of the vertebral canal occupied by the cyst on MRI and the degree of spondylolisthesis in the involved segment.…”

“…Intradural pathology and tumors of the spine can be managed by minimally invasive techniques conserving the bony and musculoligamentous tissues essential for stability of spine. There are studies reporting the usage of microtubular technique to excise focal tumors like osteoid osteoma from C2 vertebra 24,37,38) . The tubular retractors provide adequate window for tumor surgery which is still not completely established with full endoscopy.…”

“…Several studies have identified specific MRI features, such as intraspinal synovial cyst, facet fluid, and facet orientation, which may be suggestive of spondylolisthesis [3][4][5]. Extensive literature is available for the lumbar spine which states that the presence of facet fluid is correlated with the likelihood of lumbar spondylolisthesis [5][6][7][8][9][10][11].…”
