This study aims to examine the determinants of catastrophic health expenditure in households with cancer patients by conducting a panel analysis of three-year data. Data are adopted from surveys administered by Korea Health Panel for 2012–2014. We conducted correspondence and conditional transition probability analyses to examine households that incurred catastrophic health expenditure, followed by a panel logit analysis. The analyses reveal three notable results. First, the occurrence of catastrophic health expenditure differs by age group, that is, the probability of incurring catastrophic health expenditure increases with age. Second, this probability is higher in households with National Health Insurance than those receiving medical care benefits. Finally, households without private health insurance report a higher occurrence rate. The findings suggest that elderly people with cancer have greater medical coverage and healthcare needs. Private health insurance contributes toward protecting households from catastrophic health expenditure. Therefore, future research is needed on catastrophic health expenditure with focus on varying age groups, healthcare coverage type, and private health insurance.

Objetivo Describir las distintas percepciones de los derechos de salud para la población migrante en Chile publicadas en Twitter a propósito del primer trasplante hepático realizado en el país a una mujer migrante haitiana en septiembre del año 2018. Método Estudio cualitativo, análisis de caso. El caso corresponde a la primera mujer migrante receptora de un trasplante hepático de urgencia en Chile. Se recogieron opiniones vertidas en Twitter respecto a este caso entre el 29 de septiembre y el 17 de noviembre de 2018 (n = 339). Se realizó un análisis temático utilizando el software NVivo12 con códigos delineados según el objetivo. Resultados Según las percepciones planteadas en Twitter, el derecho de acceso a servicios de salud de la población migrante en Chile debiera ser limitado, y otorgar prioridad a los nacionales. Estas opiniones conviven con visualizar a la salud como un derecho humano. Aparecen también sentimientos de racismo y discriminación hacia este grupo. Conclusiones En Chile conviven distintas percepciones de cuál debiera ser la concesión de derechos de acceso a servicios de salud de las personas inmigrantes. Esto puede generar no solo un recrudecimiento de la estigmatización y la vulnerabilidad que enfrenta este grupo, sino también una barrera para la implementación de la política, agudizando aún más la presencia de inequidades en salud.

In Ukraine, there is a steady increase in the mortality rate of the population, which is due to the low quality of medical provision. The main advantages and the efficiency factor of the health care system in the Western European countries are analyzed; the key factors regulating medical provision in Ukraine are systematized; the effectiveness of modern health care reform in Ukraine is evaluated; practical recommendations to improve the administrative and legal regulation of the system are developed. The analysis showed that the effectiveness of health care reform in Ukraine is currently about 30%, and the high mortality rate of the population in Ukraine is primarily due to the low quality of medical care and inadequate investment in the industry. The main health problems in Ukraine are failure to comply with legislation on the amount of funding and remuneration in the public health system, high levels of corruption and lack of control over the competence of doctors, inaccessibility of medical products for consumers due to the pharmaceutical market monopolization.
