Should every patient diagnosed with a phaeochromocytoma have a 123IMIBG scintigraphy?

Abstract: Localization of phaeochromocytomas and paragangliomas (PPGLs) should involve functional imaging as anatomical imaging modalities can either fail to locate the tumour or can be suboptimal due to an anatomical abnormality or previous surgery. Functional imaging is particularly useful to fully delineate the extent of disease using the whole-body scan and the evaluation of multifocality, metastatic or recurrent disease. An increasing number of radiolabeled tracers have become available for tumour visualization dur… Show more

“…15 Functional imaging comes into its own in 6 particular scenarios: The smallest PC DOTATATE-PET correctly identified in primary assessment was 19 mm and secondary assessment; 10 mm (local recurrence). Previous studies have shown that MIBG is insensitive to these small PCs (reviewed in 25 ). In our series, 9/12 PCs that had both MIBG and DOTATATE-PET scans were positive only on PET.…”

“…Intracellularly, MIBG is stored in neurosecretory granules through the action of vesicular monoamine transporters (VMATs) (2,3). MIBG has proven to be useful for scintigraphic imaging of pheochromocytomas and paragangliomas (PPGLs) (4,5). These rare and often benign catecholamine-producing tumors arise from the adrenal medulla and extraadrenal sympathetic chromaffin tissues (5).…”

“…At present, scintigraphy with 123 I-MIBG is the most commonly used functional imaging technique for localizing PPGLs because of its high specificity and worldwide availability (4,5). Its whole-body exploration makes it particularly useful for the evaluation of multifocality and metastatic and recurrent disease.…”
