Should ALPPS be Used for Liver Resection in Intermediate-Stage HCC?

Abstract: The ALPPS approach induces a considerable hypertrophic response in HCC patients and allows resection of intermediate-stage HCC, albeit at the cost of a 31 % perioperative mortality rate. The use of ALPPS for HCC remains prohibitive for most patients and should be performed only for a highly selected patient population younger than 60 years with low-grade fibrosis.

“…This concept of adequate patient selection is currently also the basis of the stepwise therapy associating liver partition and portal vein ligation for staged hepatectomy (ALPPS). Because after an international registry showed a higher mortality after ALPPS in patients aged above 61 years [27], now this procedure is recommended only for selected, e.g. young patients [28].…”

“…Then, resection is performed when the future remnant liver volume is more than 30–40% of the standardized liver volume. Preoperative evaluation should include liver cirrhosis, the patient’s age and ability to tolerate two surgeries within a short period of time, and the risk of rapid tumor progression [53]. Laparoscopy or ablation can be used to minimize the trauma associated with the procedure [54].…”

“…The ALPPS procedure resulted in a quadrupled mortality rate, doubled median hospital stay, and doubled risk for PLF in patients with intermediate-stage HCC [132] and should therefore only be applied in HCC patients with low-grade fibrosis. ALPPS should not, or with great caution, be applied in patients with perihilar and intrahepatic CCA due to the (already) high postoperative mortality rate in this patient category [130,133].…”
