Should All Papillary Thyroid Microcarcinomas Be Aggressively Treated?

Abstract: Although PTMC is generally associated with an excellent prognosis, 0.5% patients may die of PTMC. The presence of 2 or more risk factors is strongly associated with cancer-related mortality and can help to identify patients who should be considered for more aggressive management.

“…If this were the case, total thyroidectomy might prove beneficial in this group. We performed a subset analysis of Black 13 Similar to our study, they compared total thyroidectomy against thyroid lobectomy in patients with two or more risk factors. Contrary to our study, they found that total thyroidectomy provided a survival advantage in this cohort of patients.…”

“…Yu et al evaluated risk factors for mortality in patients with papillary thyroid microcarcinoma. 13 The study included 18 445 patients from the Surveillance, Epidemiology, and End Results database. Similar to our study, they found that age greater than 45 years, male sex, and African American or minority race were associated with increased risk of mortality.…”

“…3 Fortunately, tumor-related mortality has remained very low, and reported to be 0.5% for microcarcinoma. 4 Despite this, the optimal management of microcarcinoma remains a subject of ongoing debate. A subset of microcarcinoma is associated with local recurrence and nodal metastasis.…”

“…A subset of microcarcinoma is associated with local recurrence and nodal metastasis. 4,5 The identification of this aggressive subset is critical to better triaging of the patients with regard to the need for additional therapy. 6 Clinicopathological factors, eg, age more than 45 years, tumor size greater than 5 mm, male sex, multifocality, lymph nodes metastasis, and extrathyroidal extension have been reported to predict poor prognosis.…”
