Background1 Despite a growing interest in gaming rehabilitation for upper limb (UL) recovery post-stroke, studies investigating the effects of game-based rehabilitation incorporating functional games are lacking. Objective To investigate the efficacy of an intensive, functional, gamified rehabilitation program compared to task-based training on UL motor function in acute/sub-acute stroke survivors. Methods This randomized, multicenter, single-blind, clinical trial comprises 120 participants with unilateral stroke who were randomized to receive either gamified training (n = 64) using the ArmAble™ [experimental group (EG)] or task-based training (n = 56) in conjunction with conventional therapy for 2 hours per day, 6 days per week for 2 weeks, followed by UL rehabilitation for another 4 weeks at home. Primary outcomes evaluated by a blinded assessor included the Fugl-Meyer Assessment-Upper Extremity (FM-UE), and Action Research Arm Test (ARAT). Data were analyzed using a linear mixed-effect regression model. Results The mean (standard deviation) age of the participants was 54.4 ± 11.7 years (78.1% men) in the EG and 57.7 ± 10.9 years (73.2% men) in the comparator group (CG). The median (interquartile range) time since stroke was 30.0 (54.0) days in the EG and 22.5 (45.0) days in the CG. Following the 2-week intervention, a statistically significant improvement was observed in the EG for the FM-UE [between-group mean differences (95% confidence interval): −3.9 (−6.5, −1.3); P = .003]; but not for the ARAT [−2.9 (−5.8, 0.0); P = .051]. Gains at 6 weeks were significantly greater in the EG for both FM-UE [−3.9 (−6.5, −1.3); P = .003]; and ARAT [−3.0 (−5.9, −0.0); P = .046]. Conclusion Gamified rehabilitation using the ArmAble™ device has shown immediate and short-term improvement in UL function after acute/sub-acute stroke. The incidence of stroke in India is one of the largest worldwide. With this scoping review, we assessed the evidence on the costs of stroke, which is essential to evaluate whether stroke care interventions are cost-effective. We adopted a healthcare sector and broader societal perspective and searched electronic databases for records including stroke cost estimates (up to 2020). Following deduplication and screening of 2,510 records, we extracted the data (converted into 2020 Indian rupees [INR]) and assessed the quality of eight eligible studies. These studies, published between 2011 and 2020, covered the whole of India and specific localities and were: cost-of-illness studies (n = 3); economic evaluations (n = 2); cross-sectional costing study (n = 1); simulated costing study (n = 1); and policy/clinical review (n = 1). Among the extracted estimates, the mean total costs of stroke care per patient for the period up to six months post admission ranged from ₹19,428.86 in a government hospital in Punjab to ₹118,040.15 in a private tertiary care centre in Ludhiana. The median total out-of-pocket (OOP) payments for cardiovascular disease-related expenditures ranged from ₹18,148.88 to ₹68,464.39 across different income groups. Despite methodological limitations, the data from our scoping review will help in designing economic evaluations of stroke care interventions in India. JEL Codes: C18, C80, D61, I15, I19

Mục tiêu: Ước tính chi phí trực tiếp dành cho điều trị nội trú của người bệnh đột quỵ nhồi máu não tại Trung tâm Thần Kinh Bệnh viện Bạch Mai, năm 2021. Đối tượng và phương pháp: Nghiên cứu 500 người bệnh (NB) bị đột quỵ nhồi máu não cấp, điều trị tại Trung tâm Thần kinh, Bệnh viện Bạch Mai từ 01/07/2021 đến 31/12/2021. Kết quả: Chi phí y tế trực tiếp dành cho điều trị nội trú của người bệnh đột quỵ nhồi máu não trung bình là 10.519 ± 7.268 nghìn đồng, trong đó chi phí cho giường bệnh cao nhất là 3.643 ± 2.980 nghìn đồng, chi phí cho thuốc: 2.839 ± 3.037 (thuốc tiêu sợi huyết chi phí nhiều nhất là 17.206±5.960) nghìn đồng. Trong các dịch vụ đã được sử dụng, chi phí cho vật tư thấp nhất 118 ± 77 nghìn đồng, chi phí cho xét nghiệm cận lâm sàng cao nhất là về hình ảnh (xquang, CT, MRI) trung bình 1.499±1.222 nghìn đồng. Có sự khác biệt về mức chi phí trung bình trong các nhóm NB có tỷ lệ được BHYT thanh toán khác nhau. NB được thanh toán BHYT 100% có chi phí trung bình là 11.842±7.977 nghìn đồng, chi phí trung vị là 9.186 nghìn đồng, cao hơn chi phí của NB có tỷ lệ được BHYT thanh toán 95%, 80%, 40% và 0% với chi phí trung bình lần lượt là 11.670±8.930 nghìn đồng, 10.743±7.217 nghìn đồng, 8.518±5.478 nghìn đồng và 9.566±6.363 nghìn đồng (p<0.05). Kết luận: Chi phí trực tiếp dành cho điều trị nội trú của người bệnh đột quỵ nhồi máu não tại Trung tâm Thần Kinh Bệnh viện Bạch Mai, năm 2021 dao động cao nhất 39.753 đến 3.326 nghìn đồng. Người bệnh BHYT tùy mức hưởng có ảnh hưởng có lợi đến sự thay đổi chi phí điều trị, có thể giảm 2,9% tổng chi phí điều trị đột quỵ nhồi máu não cấp tính.
