Short and oral antimicrobial therapy for diabetic foot infection: a narrative review of current knowledge

Abstract: Abstract. Diabetic foot infection is a frequent complication in long-standing diabetes mellitus. For antimicrobial therapy of this infection, both the optimal duration and the route of administration are often based more on expert opinion than on published evidence. We reviewed the scientific literature, specifically seeking prospective trials, and aimed at addressing two clinical issues: (1) shortening the currently recommended antibiotic duration and (2) using oral (rather than parenteral) therapy, especiall… Show more

“…We examined risk factors of osteomyelitis in patients with fractures of the upper and lower extremities. Our findings are consistent with the results of studies that have associated a history of diabetes or stress-induced hyperglycemia with infectious complications including osteomyelitis [ 13 , 14 , 15 ]. Patients with diabetes tend to have impaired bone regeneration [ 16 , 17 ].…”

“…Osteomyelitis is a specific podiatric problem typically resolved using long-term ATB therapy lasting 6-8 weeks or longer in the case of conservative management ( 12 ). Recently, however, there is a trend to reduce ATB treatment in the case of osteomyelitis, with a recommended maximum duration of 4-6 weeks or even shorter in selected cases ( 13 ).…”
