Short and long-term post-operative outcomes of duodenum preserving pancreatic head resection for chronic pancreatitis affecting the head of pancreas: a systematic review and meta-analysis

Abstract: All procedures are equally effective for the management of pain for chronic pancreatitis. The choice of procedure should be determined by other factors including the presence of secondary complications of pancreatitis and intra-operative findings. Registration number CRD42015019275. “…Nevertheless, there were no significant differences in pain reduction directly after surgery, mortality, or exocrine and endocrine pancreatic function (39). A recent analysis of five studies, including one randomized trial, agreed with us in finding no important differences among the various DPPHR procedures (40). A feature shared by all reviews published to date is that they only investigate subgroups of operation techniques (38,39,40) and potentially display increased bias because they included no randomized controlled trials (38,40).…”

“…Die einzelnen Modifikationen der DEPKR wurden mehrmals im Rahmen von RCT und Metaanalysen miteinander verglichen. Als technisch einfachste, schnellste und effektive Methode hat sich die Berner Modifikation der DEPKR durchgesetzt [32,42,43]. Auch die kürzlich publizierten langfristigen Ergebnisse 10 Jahre nach Berner und Beger-Prozeduren bevorzugen aufgrund ihrer Ergebnisse die Berner Modifikation [44].…”

“…One area of particular interest has been weighing the relative benefits of partial pancreaticoduodenectomy compared with DPPHR. To this end, a number of recent publications have investigated the role of DPPHR [3,15 ■ ,16,17]. The European ChroPac randomized controlled trial compared quality of life at 2 years postoperatively for patients randomized to pancreaticoduodenectomy and DPPHR groups at 18 hospitals [15 ■ ].…”
