Zusammenfassung Hintergrund Die Bauchlagerung („proning“) als Teil der Behandlungsstrategie bei Patient:innen mit Acute Respiratory Distress Syndrome (ARDS) hat durch die COVID-19-Pandemie massiv an Bedeutung gewonnen. Neben den positiven Effekten der BL sind Nebenwirkungen bekannt, insbesondere Druckulcera (DU) und periphere Nervenläsionen (pNL). Methoden In dieser retrospektiven Kohortenstudie wurden COVID-19-ARDS-Patient:innen, die supportiv in BL therapiert und im Behandlungsverlauf auf die Frührehabilitation übernommen wurden, auf das Vorliegen von DU und pNL hin untersucht. Es wurden potenzielle Risikofaktoren für das Entstehen von DU und pNL und der Einfluss von DU und pNL auf den Rehabilitationserfolg explorativ analysiert. Ergebnisse Insgesamt wurden 52 Patient:innen in die Studie eingeschlossen. Von ihnen boten 10 (19,2%) das klinische Bild einer pNL und 41 (78,8%) wiesen bei Übernahme Bauchlagerungs-typische ventrale DU auf. Patient:innen mit pNL hatten signifikant häufiger einen Diabetes mellitus als Vorerkrankung, für die Entstehung von DU gab es keine Risikofaktoren. Bei Übernahme waren Patient:innen mit pNL im Durchschnitt weniger mobil und weniger selbstständig als Patient:innen ohne pNL, während zum Zeitpunkt der Entlassung keine signifikanten Unterschiede in Mobilität und ADL-Fähigkeit mehr vorlagen. Patient:innen mit pNL benötigten signifikant mehr Hilfsmittel bei Entlassung. DU sind häufig, v. a. im Gesicht-/Halsbereich (56,1%). Schlussfolgerungen Nebenwirkungen der Bauchlagerung wie DU und pNL sind zum Zeitpunkt der Übernahme in die Frührehabilitation häufig. Während die Mobilität zum Übernahmezeitpunkt durch das Vorliegen einer pNL deutlicher eingeschränkt ist, besteht dieser Unterschied nicht mehr zum Entlasszeitpunkt.

Objetivo: Avaliar o impacto da pandemia de COVID-19 na incidência de lesão por pressão em pacientes críticos, bem como descrever as características dessas lesões e o perfil dos pacientes acometidos. Método: trata-se de uma revisão sistemática da literatura, realizada conforme as diretrizes PRISMA, pesquisada nas bases de dados MEDLINE, Scopus, Literatura Latino-Americana e do Caribe em Ciências da Saúde, EMBASE e Web of Science. Resultados: Dos 339 estudos inicialmente identificados, 10 atenderam aos critérios de inclusão e foram analisados. A incidência de lesão por pressão variou de 33% a 80%, com uma mediana de 54% (IQR 43% - 73%). Seis estudos avaliaram a incidência de lesão por pressão exclusivamente em pacientes submetidos a posição prona; nestes estudos a incidência de lesão por pressão variou entre 40% a 77%, mantendo a mediana de 54% (IQR 47% - 72%). A maior parte das lesões documentadas era em estágio II (30% IQR 23%-69%). Em relação ao local da lesão, a maior prevalência é em face/rosto em decorrência do posicionamento prona e na região sacra predominante nos estudos que avaliam a incidência de lesão por pressão no geral. Conclusões: A pandemia de COVID-19 teve impacto substancial na incidência de lesão por pressão em pacientes sob cuidados intensivos, impulsionado pelo influxo abrupto de casos da doença no início da pandemia, o que dificultou a organização das instituições no planejamento das ações de cuidado e prevenção dessa injúria.

In patients with COVID‐19 different methods improving therapy have been used, including one of the anatomical position–prone position, to support ventilation. The aim of this review was to summarize the cases of brachial plexopathy as a consequence of the prone position in COVID‐19 patients, and thus bring closer the issue of the brachial plexus in the face of clinical aspects of its function, palsy, and consequences. The Preferred Reporting Items for Systematic Reviews and Meta‐analyses statement was followed, inclusion criteria were created according to Patients, Interventions, Comparisons, Outcomes (PICO). PubMed and Scopus were searched until April 1, 2023 by entering the key term with Boolean terms. The risk of bias was assessed using JBI's critical appraisal tools. Fifteen papers with 30 patients were included in the review. This study showed that brachial plexopathy after the prone position occurs more often among males, who are at least 50 years old with comorbidities like hypertension, overweight, and diabetes mellitus. The most common symptoms were weakness, pain, and motion deficits. Duration of the prone position session and the number of episodes were different as well as the modification of positioning. Brachial plexopathy is a significant problem during prone position, especially when hospitalization is prolonged, patients are males, have comorbidities, and changes in body weight. Attention should be drawn to understand the anatomy of the brachial plexus, correct positioning, avoiding factors worsening the prognosis, and proper nutrition of the patients.
