Short- and intermediate-term results of balloon aortic valvuloplasty and surgical aortic valvotomy in neonates

Abstract: Background:Balloon aortic valvuloplasty and open surgical valvotomy are procedures to treat neonatal aortic stenosis, and there is controversy as to which method has superior outcomes. Methods:We reviewed the records of patients at our institution since 2000 who had a balloon aortic valvuloplasty or surgical valvotomy via an open commissurotomy prior to 2 months of age. Results:Forty patients had balloon aortic valvuloplasty and 15 patients had surgical valvotomy via an open commissurotomy. There was no differen… Show more

“…7,9 11 Recently, our institution evaluated initial SAV and BAD approaches in patients ≤60 days of age and found similar reductions in valve gradients postprocedure but more significant and more frequent aortic insufficiency (AI) at up to 5 years in the BAD group. 12 For the present study, we aimed to evaluate longer term outcomes following surgical valvuloplasty and balloon dilation in pediatric patients >60 days old with congenital AS.…”

“…1 There is no generally accepted definition of critical VAS. In the literature, the definitions includes: age at intervention, <30 days 2 3 or <3 months [4][5][6][7] ; anatomic features such as duct dependency 8 9 ; functional features such as depressed left ventricular function 10 ; ongoing pharmacological treatment with prostaglandin or inotrope support 11 12 ; or as a combination of criteria. 1 13-15 Another group of neonates with VAS present in good clinical condition with a heart murmur, and the indication for treatment in the neonatal period is a significant gradient across the aortic valve.…”

“…11,12 Aortic valve insufficiency may be well-tolerated acutely, but over time is progressive, and in the setting of left ventricular dilatation, can by itself be an indication for aortic valve replacement or surgical palliation. 13,14 As valve insufficiency remains a common complication of aortic valvuloplasty that can significantly influence long-term outcomes, 5,15,16 advances that reduce valve insufficiency but preserve gradient reduction should continuously be pursued.…”
