Short and inflamed cervix predicts spontaneous preterm birth (COLIBRI study)

Abstract: COLIBRI study used a novel, single-step method of vaginal secretions sampling during TVUS and demonstrated that combination of short cervix and IL-8 in vaginal secretions is a promising sPTB predictive test.

“…This included significant differences in IL-6, IL-8, IL-10, TNF-α, and CRP between women who delivered preterm versus full-term. This is consistent with previous research findings on higher levels of CVF IL-8 associated with gestational age, 12 spontaneous preterm birth, 13 and, among women with preterm labor and without PPROM, delivery within 7 days. 14 One study found no association between CVF IL-8 and cervical shortening at 20 or 24 weeks' gestation; 15 given the difference between this result and our present findings, changes in IL-8 may be better detected later in pregnancy, or may be associated with other symptoms of preterm birth.…”

“…Similar to the previous studies, 2 , 18 , 22 , 30 we found that individual cytokines in CVF and sonographic cervical length have an independent relation in predicting delivery within 7 days, and thus multiple marker combinations have better overall diagnostic performance than any of the individual markers. In this respect, a combination of CVF IL‐8 and cervical length is the best prediction test combination for impending preterm delivery, yielding the highest specificity (92%), although its sensitivity (56%) is not quite good.…”

“…Interleukin-8 (IL-8) is a chemokine that targets neutrophils to promote their chemotaxis and activation as well as induce reactive oxygen species (ROS) (5). Its presence in cervico-vaginal fl uid has been associated with cervical dilation, preterm labor, labor at term, cervico-vaginal infection, amniotic fl uid infection, chorioamnionitis and adverse cerclage outcome (6)(7)(8)(9)(10)(11). The inducible 70kDa heat shock protein (HSPA1A), commonly known as hsp70, is a protein produced in response of stress.…”
