Sex hormone binding globulin (SHBG) is known as a carrier protein, classically thought to be mainly synthesized in the liver and then secreted into the circulating system, where it binds to sex steroids with a high affinity and modulates the bioavailability of these hormones. In humans, the organs other than the liver known to produce SHBG include the brain, uterus, testis, prostate, breast and ovary, and the locally expressed SHBG is considered to play an important role in various physiological and pathological processes. A few studies of SHBG in rats were reported, but systemic SHBG studies in consideration of different organs and aging are currently missing. So we examined the SHBG expression in the brain, liver, prostate, and serum in 40 Sprague-Dawley male rats in four different groups (newborn, 2, 6, and 12 months old, respectively) with 10 in each group by immunohistochemistry, immunofluorescience microscopy, qRT-PCR, ELISA, western blotting, and laser confocal microscopy. We discovered that SHBG was increasingly expressed in all the three tissues along with age, and the SHBG protein expression was observed in the cytoplasm and membrane of neurons, hepatocyte, and prostate epithelial cells. The ELISA assay of the sera also supported an increasing SHBG level along with age. It is concluded that the locally synthesized SHBG in the liver, brain, and prostate and the circulating SHBG of male SD rats are positively associated with age, further indicating a potential role of SHBG in aging.

The role of serum uric acid (SUA), an inflammatory agent and potential mediator of cardiovascular diseases, in endothelial function (EF) has been tested only in middle-aged subjects affected by specific diseases. Our aim was to assess the relationship between SUA and measures of EF in a cohort of elderly community-dwellers. This study involved 424 males and 426 females aged 70 years from the Prospective Study of the Vasculature in Uppsala Seniors (PIVUS), having complete data on SUA and EF assessed by flow-mediated vasodilation (FMD) and by intra-arterial infusion of acetylcholine (endothelium-dependent vasodilation, EDV) and sodium nitroprusside (endothelium-independent vasodilation, EIDV). Univariate and multivariate regression models obtained by backward selection from initial fully-adjusted models were built to assess the relationship between SUA and measures of EF in both genders. Cardiovascular risk factors, serum hormonal and metabolic mediators, and body composition were considered as potential confounders. In the univariate model, SUA was inversely associated in both genders with log(EDV) (β ± SE males −0.39 ± 0.17, p = 0.03; females −0.57 ± 0.19, p = 0.003) and log(EIDV) (males −0.23 ± 0.12, p = 0.05; females −0.49 ± 0.15, p = 0.002), but not with log(FMD). After adjustment for BMI, only the association between SUA and log(EIDV) in females persisted, though attenuated (−0.32 ± 0.16, p = 0.049), and was no longer significant in the fully-adjusted multivariate model including waist/hip ratio. In conclusion, in older subjects, especially women, SUA is associated with EF not independently of a list of confounders including BMI and trunk fat mass, suggesting a role as surrogate metabolic marker rather than an active player in EF.

Current medical management of endocrine-related male infertility

Male factor contributes to 50%–60% of overall infertility but is solely responsible in only 20% of couples. Although most male factor infertility is ascertained from an abnormal semen analysis, other male factors can be contributory especially if the sample returns normal. Male infertility can be due to identifiable hormonal or anatomical etiologies that may be reversible or irreversible. This manuscript will highlight existing guidelines and our recommendations for hormone evaluation for male infertility and empiric therapies including multivitamins, estrogen receptor modulators (clomiphene), estrogen conversion blockers (anastrozole), and hormone replacement.
