Sexual dimorphism in cortisol secretion starts after age 10 in healthy children: urinary cortisol metabolite excretion rates during growth

Abstract: Wudy SA, Hartmann MF, Remer T. Sexual dimorphism in cortisol secretion starts after age 10 in healthy children: urinary cortisol metabolite excretion rates during growth. Am J Physiol Endocrinol Metab 293: E970-E976, 2007. First published July 17, 2007; doi:10.1152/ajpendo.00495.2006.-Detailed data on the physiological pattern of adrenocortical activity during normal growth are lacking. An established method to determine adrenocortical glucocorticoid secretion is the measurement of 24-h excretion rates of maj… Show more

“…Factors that have been demonstrated previously to affect cortisol values including gender, BMI and inhaled corticosteroid use did not influence the downward trend in peak cortisol responses in adolescence. Sexual dimorphism has been demonstrated previously with respect to cortisol production and metabolism in adolescents (30) and older adults (31,32). In our cohort, there was a non-significant tendency towards higher stimulated cortisol responses in females.…”

“…No such gender difference in cortisol response to GST was found in previous studies of pediatric cohorts [6,24]. Sexual dimorphism has been reported previously with respect to cortisol production and metabolism in adolescents [29] and older adults [30], with most adult studies showing higher levels in males. In a study of healthy kindergarten children, girls exhibited an increased baseline and stress-elicited HPA system activity [31].…”

“…Today, we know that cortisol excretion rates, measurable in 24-hour urine samples, vary significantly during childhood and adolescence [29]. The onset of increased adrenal C19 steroid secretion, previously thought to occur at about 6-8 years of age, is now known to begin much earlier [2] Maybe it is time to reconsider and re-evaluate this issue.…”

“…This is explained by the inclusion of patients who were, for the most part, already treated with oral doses of HC (n = 9) and only a few HC-untreated patients (n = 4). Wudy et al [29] determined cortisol excretion rates in healthy children and adolescents aged 3-4 years and generated normal values applying exactly the same method as in the present study. Compared with the published data, the urinary excretion rates for cortisol metabolites of the untreated HP patients are clearly lower whereas those of the treated HP patients are mostly within the given range, indicating sufficient therapy.…”
