Objective Main aim of this study was to evaluate circulating selenoprotein P (SEPP) levels in patients with simple steatosis (SS) and nonalcoholic steatohepatitis (NASH) compared with healthy controls. Methods Thirty-one patients with biopsy-proven NAFLD (15 with SS, 10 with borderline NASH, 6 with definite NASH) and 27 matched controls without NAFLD were enrolled. Serum SEPP levels and liver function tests plus biochemical parameters were measured with ELISA and standard methods, respectively. Homeostatic model of assessment - insulin resistance (HOMA-IR) was calculated. Results SEPP levels were statistically different between groups (p-value for trend=0.043). In pairwise comparisons, SEPP was lower in definite NASH compared with controls (p=0.029), but not SS (p=0.18) or borderline NASH (p=0.35). SEPP was not different between controls, SS and borderline NASH. The unadjusted trend between the controls, SS and NASH patients remained essentially unchanged after adjustment for age, sex, log(ALT) and waist circumference, but it marginally lost significance when log(HOMA-IR) entered into the model. SEPP levels were not different between groups of different severity of steatosis, fibrosis, hepatocellular ballooning, lobular and portal inflammation. Conclusions Lower SEPP levels were observed in patients with definite NASH compared with controls, a finding warranting larger studies.

Selenium (Se) shows biologically ambivalent characteristics in animals. It is an essential element but becomes severely toxic when the amount ingested exceeds the adequate intake level. Its biological, nutritional, and toxicological effects are strongly dependent on its chemical form. In this study, we evaluated the toxicity and bioavailability of nine naturally occurring Se compounds, or the so-called bioselenocompounds, in vivo and in vitro. Selenite and selenocystine showed higher toxicity than the other bioselenocompounds in vitro. In an in vitro membrane permeability study using Caco-2 cells, selenomethionine and Se-methylselenocysteine were more efficiently transported than the other bioselenocompounds. The effect of bioselenocompounds on nutritional availability was quantitatively determined from the recovery of serum selenoproteins in Se-deficient rats by speciation analysis. In contrast to the in vitro study, there were no significant differences in the assimilation of Se into serum selenoproteins among the bioselenocompounds, including selenoamino acids, selenosugar, and inorganic Se species, such as selenite, selenate, and selenocyanate, except trimethylselenonium ion. These results indicate that animals can equally assimilate both inorganic and organic naturally occurring selenocompounds except trimethylselenonium ion, which is the urinary metabolite of excess Se. We confirmed that the bioselenocompounds except trimethylselenonium ion had equivalent nutritional availabilities.

ZusammenfassungImmer mehr Untersuchungen legen nahe, dass ein ausreichend hoher Selenspiegel bei Tumorpatienten dazu beiträgt, die Chancen auf eine günstige Prognose zu erhöhen. Da eine Krebserkrankung häufig mit einem Selendefizit assoziiert ist, das durch die notwendige tumorreduktive Therapie meist noch verstärkt wird, liegt es nahe, das essenzielle Spurenelement Selen zu supplementieren. Ein praktisches Behandlungsproblem besteht dabei in der Wahl der geeigneten Dosierung. Viele Labors verwenden unterschiedliche Referenzbereiche und teilweise auch nicht-valide Messmethoden. Der Optimalwert kann bisher nur aus verschiedenen Publikationen abgeschätzt werden. Außerdem beeinflussen Komorbiditäten den akuten Selenbedarf. Streng genommen sollte somit die Selendosierung für jeden Patienten individuell eingestellt werden; Richtwerte aus der Praxis geben allerdings Anhaltspunkte für eine adäquate Dosierung. Im Beitrag wird versucht, auf häufig gestellte Fragen aus der täglichen Beratungspraxis Antworten zu geben.
