Sex Differences in Patient-Reported Outcomes After Anterior Cruciate Ligament Reconstruction

Abstract: Female patients reported statistically significant worse outcomes than male patients before and at 1 and 2 years after ACL reconstruction. In some age groups, this difference was also clinically relevant. There were no clinically relevant sex differences in improvements over time. We suggest that possible sex differences be analyzed in future studies on evaluation after ACL injury/reconstruction.

“…Despite the well-described role of sex on ACL injury risk, the role of sex on ACL injury treatment outcomes has been a topic of considerable debate with inconclusive findings [1,43,47,51]. The discrepancy in these results has been attributed mainly to the challenges associated with clinical studies of human cohorts, including relatively short followup and lack of outcome measures with sufficient sensitivity to detect sex-specific differences.…”

“…Some results based on data from the register have previously been published. 1,2 Similar registers exist in other countries, such as the Danish 3 and Norwegian registers, 4 as well as the Multicenter Orthopedic Outcomes Network study (MOON study). 5 In addition, databases like the Kaiser Permanente Anterior Cruciate Ligament Reconstruction Registry (KPACLRR) 6 and the New York database 7 exist but without patient-reported outcomes.…”

“…The injury meniscus was the main concomitant lesion found in this study and has a prevalence of 41% to 82% [3], this may arise as a result of instability of the knee after the injury and may recur if the instability remains [18].…”

“…The higher prevalence of injuries in females is attributed to biomechanical and structural factors [4,8,15] that result in worse outcomes of injuries before ACL reconstruction surgery when compared to men [3].…”
