Severity of statin-induced adverse effects on muscle and associated conditions: data from the DAMA study

Abstract: Myalgia was the most common muscle adverse effect associated with statin therapy. Excessive alcohol consumption and pravastatin were independently associated with myositis or rhabdomyolysis.

“…At present, the most prescribed medications for hyperlipidemia in diabetes include Fibrates and Stains. However, side effects of Fibrates and Stains [7][8][9] makes it significant to develop functional foods-and natural products-based treatments which could be safer and more tolerable for patients.…”

“…25 Statins, HMG-CoA reductase inhibitors, are the most widely used medications for decreasing LDL cholesterol and play a vital role in the prevention of atherosclerotic cardiovascular complications. 19 In the present study, simvastatin, atorvastatin and lovastatin were prescribed for 29 (38.7%), 28 (37.3%) and 15 (20%) of the total 75 study participants, respectively.…”

“…The mechanism of statin-induced myopathy could be impaired synthesis of cholesterol leading to changes in the cholesterol in myocyte membranes and behavior of the membrane, impaired synthesis of compounds in the cholesterol pathway (coenzyme Q10) and depletion of isoprenoids, which prevents myofibril apoptosis. [18][19][20] Measuring serum CK is an important part of the evaluation of patients with muscle weakness or myalgia, and assessing of patients with myopathies or muscle injuries. 21 Similarly, evaluation of serum LDH or LDH isoenzyme is relevant in the diagnosis, prognosis and monitoring of diseases such as myocardial infarction, hemolytic anemia, hepatocellular carcinoma, ovarian dysgerminoma and testicular germ cell tumor.…”

“…The most common adverse effect of statins are musculoskeletal events including myopathy, myalgias, muscle weakness, arthropathies, and, rarely, rhabdomyolysis [ 13 , 14 , 15 ]. Estimates of the number needed to harm (NNH) in retrospective studies have ranged from 19 to 47 for musculoskeletal disorders [ 7 , 16 , 17 ], and statins markedly increase the risk of myopathy [odds ratio (OR): 2.63, 95% confidence interval (CI): 1.50–4.61] [ 18 ].…”

“…Canagliflozin is a diabetes drug and transport inhibitor that enhances statin absorption; the patient’s plasma rosuvastatin concentration was 15-fold higher than expected for a 40 mg dose, and the case resolved once the two drugs were stopped. While high-intensity statins can be avoided to reduce the severity of adverse effects, muscle symptoms are also reported with moderate-intensity therapy [ 14 ]. Muscle weakness and pain pose a particular problem for the elderly, in whom statins might contribute to deconditioning, frailty, and falls [ 21 ].…”
