Severity and Outcomes of Dengue in Hospitalized Jamaican Children in 2018–2019 During an Epidemic Surge in the Americas

Abstract: ObjectiveIn 2019, dengue was among the “top-ten threats to global health,” with 3.1 million cases reported from the Americas, the highest ever. Simultaneously, Jamaica reported its largest dengue outbreak in 40 years, following Chikungunya and Zika virus epidemics, in 2014 and 2016–2017, respectively. We describe dengue in children admitted to five hospitals in Jamaica during August 2018 through September 2019.MethodsHospitalized children and adolescents aged 0 to 15 years with dengue were managed using PAHO/W… Show more

Jamaica has reported trends of dengue in children over the years [ 16 , 18 ▪▪ , 19 – 21 , 24 ▪ ]. During the 2018–2019 epidemic, Jamaican children were most significantly affected with increased attributable morbidity and mortality [ 16 , 18 ▪▪ , 24 ▪ ].…”

“…Jamaica has experienced several DENV outbreaks of increasing intensity, severity, and magnitude in recent years (2010, 2012, and 2018-2019), with increased attributable morbidity and mortality in the very young, including reported cases of neonatal microcephaly [11][12][13][14]. Jamaica also experienced an explosive CHIKV epidemic in 2014 [3,[15][16][17][18]: >80% of the general and antenatal populations were affected, with significant perinatal (maternal and newborn) illnesses, increased attributable neonatal morbidity and mortality [3,16,19], and >2500 deaths during the epidemic year nationally [18].…”

“…Several markers including sickle cell disease (haemoglobin SC and SS), change in haematological parameters (decreased platelets, leucopenia), elevated levels of hepatic enzymes (transaminases, creatine kinase) have been associated with dengue haemorrhagic fever, dengue shock and mortality [12][13][14][15][16][17]. However, reliable predictive biomarkers of cardiac impairment as predictor of SD are lacking and remains a challenge in the clinical management of febrile dengue patients.…”
